隐源性肝癌与病毒性肝癌临床特征比较

目的:比较病毒性肝癌与隐源性肝癌的临床特点,尝试为隐源性肝癌的临床辨识及中西医诊治提供借鉴或参考。方法:回顾性分析比较92例病毒性肝癌患者(病毒性肝癌组)与36例隐源性肝癌患者(隐源性肝癌组)的临床资料,对两组患者的一般情况、化验指标、首发症状及中医证型进行统计分析。结果:两组患者在年龄、性别、甘油三脂(TG)、γ-谷氨酰转肽酶(γ-GGT)、碱性磷酸酶(ALP)、纤维化指数-4(FIB-4)、APRI及首发症状方面差异均有统计学意义(均P<0.05),两组患者中医证型差异无统计学意义。结论:隐源性肝癌患者多以女性为主,年龄偏高,肝硬化发生率显著低于病毒性肝癌患者,隐源性肝癌患者以手术切除的比例较高,与代谢因素密切相关,检测肝纤维化指标对于隐源性肝癌的诊断或许具有鉴别意义。

Preliminary study on clinical features of cryptogenic liver cancer with primary liver cancer

Objective:To compare the clinical features of viral liver cancer and cryptogenic liver cancer,and to provide evidence or reference for the clinical identification of cryptogenic liver cancer and the diagnosis and treatment of Chinese and Western medicine.Methods:A retrospective analysis of 92 cases of viral liver cancer patients and 36 cases of cryptogenic liver cancer patients with clinical data(general conditions,laboratory indicators,first symptoms and TCM syndrome types),using SPSS20.0 statistical software for statistics.Results:There were significant differences in age,gender,blood lipid(TG),liver function(GGT,ALP),liver fibrosis index(FIB-4,APRI)and initial symptoms(P<0.05).There was no statistical difference in TCM syndromes.Conclusion:Most of the patients with cryptogenic liver cancer are mainly female,whose age are higher.The incidence of cirrhosis is significantly lower than that of patients with viral liver cancer.So far,patients with cryptogenic liver cancer have a higher rate of surgical resection.Cryptogenic liver cancer is closely related to metabolic factors.Detection of liver fibrosis indicators may have a differential diagnosis for the diagnosis of cryptogenic liver cancer.
