比较超声心动图与心导管造影测量动脉导管未闭管径和肺动脉压

摘    要:目的探讨经胸超声心动图(transthoracic echocardiogram,TTE)与心导管造影测量动脉导管未闭(patent duetus arteriosus,PDA)管径和肺动脉压力。方法PDA61例,应用TTE诊断,测量其最窄内径和主动脉端内径,应用连续多普勒获取PDA两端压力差估测主肺动脉压,将TTE结果与PDA封堵术前心导管造影测量结果进行比较。结果TTE测量的PDA最窄内径和主动脉端内径,PDA两端压力差和主肺动脉压力的估测值与对应导管测量值呈正相关(P〈0.05)。PDA最窄内径的超声测值高于导管造影测值,而超声估测的主肺动脉压力低于导管测值(P〈0.05)。结论TTE与心导管造影在PDA管径和肺动脉压力的定量测量方面相关性好,但有一定差异,总结差异可更好地指导临床评估PDA合并肺动脉高压和介入治疗封堵器的选择。

Abstract:Objective To Evaluate the relevance between transthoracic echocardiogram (TTE) to heart catheterization in measuring the diameter of patent ductus arteriosus and assessing the pulmonary artery pressure. Methods The diameter of PDA in 61 patients and the pulmonary artery pressure were observed by TTE and heart catheterization. Results These diameters of PDA and pulmonary artery pressure measurements by TTE and heart catheterization showed excellent correlation (P < 0. 05). The narrowest diameters of PDA measured by TTE were significantly higher than measured by aortic angiography in heart catheterization (P < 0. 05). But the pulmonary artery pressure assessed by TTE were significantly lower than measured by heart catheterization (P < 0. 05). Conclusions The diameter of PDA and pulmonary artery pressure measurements by TTE and heart catheterization showed excellent correlation, but there was difference between them. TTE plays an important role in selecting patient s and evaluating the effeciency of PDA occlusion.
