云南省239例艾滋病病例分析

目的 探索云南省艾滋病病人的临床特点和就医行为,为艾滋病病人的临床治疗和关怀提供基础资料。方法 对1990～1998年云南省在医务人员随访和病人就诊过程中诊断及报告的239例AIDS病人的流行病学资料进行统计分析。结果 84％的AIDS病人通过共用针具静脉吸毒而感染,临床表现以发热(70％)、肺部感染(60％)和腹泻(49％)为主,病人从发现感染到发病的平均时间为3年,55.2％的病人在出现机会性感染后一年内死亡,病人就诊率低,94％的病人在家中死亡。结论 胃肠道和肺部感染为云南省AIDS病人的主要临床谱,艾滋病病人的就诊率低,病例报告存在医院的漏诊和漏报,应继续加强对病例报告的管理和对AIDS病人的关怀力度。

Analysis of 239 AIDS Cases Reported in Yunnan

To understand the clinic characteristics and medical care - seeking behavior of AIDS patients in Yunnan, and provide the basic information for improving the treatment and care of AIDS patients. Methods The epidemio-logical data of 239 AIDS cases reported through follow up and hospitals from 1990 to 1998 in Yunnan had been analyzed. Results 84% of the AIDS patients acquired HIV through sharing injection equipment, and the common symptoms and the complications of them were fever (70% ), pneumonia (60% ) and diarrhea (49% ) . On an average the duration from confirming HIV positive to emerging clinic symptoms among them was 3 years, and 55.2% of them died within one year after opportunistic infections. Conclusion The major disease patterns of AIDS patients reported in Yunnan are gastroenteritis and respiratory infections. The constraints such things as poorly medical care seeking behavior of AIDS patientis and the missed diagnosis and the missed case report of hospitals so commonly exist in Yunnan that the management of AIDS case report and its care initiatives should be strengthened.
