叙事医学在心内科临床规范化培训中的应用研究

摘    要:目的探讨将叙事医学干预运用到心内科临床规范化培训中的效果。方法将28名在心血管内科参加临床规范化培训的住院医师分为两组,对照组进行常规规范化培训,试验组进行常规规范化培训+叙事医学教育,通过问卷调查评估其对叙事医学的掌握情况。然后对照组在日常医疗工作中采用常规医疗措施,试验组在日常医疗工作中采用常规医疗+叙事医学联合措施,通过比较两组住院医师管理的冠脉造影患者负面情绪的异常来评价教学效果。结果经过叙事教育,试验组对叙事医学的掌握优于对照组(P<0.01),试验组患者冠脉造影术后焦虑、抑郁评分低于对照组(P<0.01)。结论对临床规范化培训住院医师进行叙事医学教育有助于提高住院医师的叙事能力,并能明显改善冠脉造影带给患者的负面情绪。

Abstract:Objective To explore the effect of narrative medicine intervention in clinical standardized training of cardiovascular department.Methods 28 residents who participated in clinical standardization training in cardiovascular medicine were divided into two groups.The control group received routine standardization training,the experimental group received routine standardization training+narrative medicine education,and their mastery of narrative medicine was evaluated by questionnaire survey.Then the control group adopted the routine medical measures in the daily medical work,the experimental group adopted the combined measures of routine medical treatment and narrative medicine in the daily medical work,and evaluated the teaching effect by comparing the abnormal negative emotions of patients with coronary angiography.Results The narrative ability of the trainees in the experimental group was better than that in the control group(P<0.01),and the scores of anxiety and depression in the experimental group were lower than that in the control group(P<0.01).Conclusion The education of narrative medicine to the trainees of clinical standardization training is helpful to improve the narrative ability of the trainees,and can obviously improve the negative emotions of the patients brought by coronary angiography.
