机械通气治疗高龄重度急性左心衰竭呼吸衰竭的效果

摘    要:目的探讨对高龄重度急性左心衰竭呼吸衰竭患者拟定机械通气治疗方案并应用后获得的临床效果。方法选择本院2017年4月—2019年7月收治的62例高龄重度急性左心衰竭呼吸衰竭患者作为试验对象;依据呼吸衰竭类型的不同完成分组;均拟定机械通气方案展开疾病治疗;A1组(25例):呼吸衰竭类型为Ⅰ型;A2组(37例):呼吸衰竭类型为Ⅱ型;就组间血气分析指标结果以及并发症总占比展开对比。结果治疗前,A1组PaCO2水平为(71.46±18.25)mmHg,PaO2水平为(50.45±12.23)mmHg;治疗后分别为(46.37±14.19)mmHg以及(84.35±22.26)mmHg;治疗前,A2组PaCO2水平为(70.35±12.55)mmHg,PaO2水平为(49.42±15.15)mmHg;治疗后分别为(48.83±12.45)mmHg以及(82.35±28.35)mmHg;治疗前,A1组高龄重度急性左心衰竭呼吸衰竭患者血气分析指标同A2组比较差异无统计学意义(P>0.05);治疗后,A1组PaCO2水平以及PaO2水平同A2组比较差异无统计学意义(P>0.05);A1组高龄重度急性左心衰竭呼吸衰竭患者并发症总占比(8.00%)明显低于A2组(45.95%,P<0.05)。结论本次研究发现,高龄重度急性左心衰竭呼吸衰竭患者于临床接受机械通气治疗期间,随着呼吸衰竭分级的增加,患者表现出并发症的概率呈现出一定程度增加,对此临床针对高龄重度急性左心衰竭呼吸衰竭患者需要做到早期诊断以及治疗,以使得患者预后显著提升,提高病情转归效果。

Effect of Mechanical Ventilation on Respiratory Failure in Elderly Patients With Severe Acute Left Heart Failure

Abstract:Objective To explore the clinical effect of mechanical ventilation in elderly patients with severe acute left heart failure and respiratory failure.Methods 62 elderly patients with severe acute left heart failure and respiratory failure admitted to our hospital from April 2017 to July 2019 were selected as subjects.Complete grouping according to different types of respiratory failure;All patients were treated with mechanical ventilation.A1 group(25 cases),respiratory failure type isⅠtype;A2 group(37 cases),respiratory failure type isⅡtype;The results of blood gas analysis and the total proportion of complications were compared.Results Before treatment,PaCO2 level in group A1 was(71.46±18.25)mmHg,and PaO2 level was(50.45±12.23)mmHg.After treatment,they were(46.37±14.19)mmHg and(84.35±22.26)mmHg,respectively.Before treatment,PaCO2 level in A2 group was(70.35±12.55)mmHg,and PaO2 level was(49.42±15.15)mmHg.After treatment,they were(48.83±12.45)mmHg and(82.35±28.35)mmHg,respectively.Before treatment,there was no significant difference in blood gas analysis index between group A1 and group A2 in elderly patients with severe acute left heart failure and respiratory failure(P>0.05).After treatment,PaCO2 level and PaO2 level in group A1 were not significantly different from that in group A2(P>0.05).The proportion of complications in group A1(8.00%)was significantly lower than that in group A2(45.95%)(P<0.05).Conclusion This study found that older patients with severe acute left heart failure,respiratory failure in clinical during mechanical ventilation therapy,with the increase of respiratory failure classification,patients show the probability of complications appear a certain degree,the clinical for senile severe acute left heart failure patients with respiratory failure to do early diagnosis and treatment,the prognosis of patients with to make a significant boost,improve the effect of disease outcome.
