265例艾滋病患者抗病毒治疗效果分析

目的评价265例艾滋病（AIDS）患者接受免费抗病毒治疗即高效抗逆转录病毒治疗（HAART）的效果，为进一步研究提高抗病毒治疗效果提供依据。方法收集2008年1月～2011年12月在我科艾滋病门诊接受HAART的265例患者的临床资料并进行随访至2012年12月，对其治疗前后的各项指标进行统计分析。结果265例患者中，死亡37例（13．96％），失访12例（4．52％），停药30例（11．32％），转诊1例（0．38％）。在治的185例患者CD4^＋淋巴细胞计数在治疗前后比较差异有统计学意义；病毒载量（HIVRNA）〈50copies／ml的患者比例逐年上升，2012年HIVRNA〈50copies／ml的患者占检测病例的91％（166／182）；在治患者依从性好，90％患者的依从性〉95％；药物不良反应较多见，其中消化道反应、骨髓抑制、肝损害、疲倦、中枢神经系统症状、药疹是最常见的不良反应。结论高效的联合抗病毒治疗对艾滋病患者有确切疗效。科学规范的督导管理，确保患者95％以上的依从性是治疗成功的关键。

EFFICACY ANALYSIS OF ANTIVIRAL THERAPY AMONG 265 AIDS PATIENTS

Objective To evaluate the efficacy of national free antiretroviral treatment on 265 AIDS patients providing the gist to improve the effect of antiretroviral treatment. Methods Clinical data of 265 AIDS patients who received highly active antiretroviral treatemnt （HAART）from 2008 to 2011 were collected and the patients were followed up to Dec. 2012. All index before and after treatment were compared and analyzed statistically. Results In 265 patients, 37 cases died （ 13.96% ）, 12 cases were lost to follow - up （4.52%）, 30 patients （ 11.32% ） discontinued the therapy, and referral in 1 case （0.38%）. In the governance of CD4 ＋ T lymphocyte count in 185 patients before and after the treatment the difference was statistically significant; The viral load （HIVRNA） 〈 50 eopies/ml patients with the proportion increased year by year, 2012 HIVRNA 〈 50 copies/ml patients accounted for 91% of test cases （166/182） ; In treated patients compliance is good, 90% of patients compliance 〉 95% ; Adverse drug reactions more often seen with gastrointestinal tract reaction, bone marrow suppression, liver damage, fatigue, central nervous system symptoms, drug eruption is one of the most common adverse reactions. Conclusion Highly effective and combined antivirus treatment was effective to AIDS patients. Scientific and standard supervision management which could ensure 95% of compliance was the key to successful treatment.
