胎膜早破残余羊水量对妊娠结局的影响

目的探讨胎膜早破不同残余羊水量对妊娠结局的影响。方法对148例足月胎膜早破孕妇资料进行回顾性分析。其中羊水量正常者53例(羊水量正常组),羊水量偏少63例(羊水量偏少组),羊水量过少32例(羊水量过少组)。结果羊水过少组剖宫产率63.13%,明显高于羊水正常组24.53%及羊水偏少组38.10%,羊水过少组与羊水正常组比较存在显著性差异(2=7.15,p<0.05);阴道自然分娩率羊水正常组为69.81%,羊水偏少组为46.03%,羊水过少组为28.13%,羊水偏少组、羊水过少组与羊水正常组之间比较存在显著性差异(2=6.64,2=13.96,p<0.05);宫内感染发生率羊水正常组0.00%,羊水偏少组3.18%,羊水过少组12.50%,在三组之间比较存在显著性差异(2=8.23,p<0.05);胎儿宫内窘迫发生率羊水正常组11.32%,羊水偏少组20.64%,羊水过少组为37.50%,羊水过少组与羊水正常组之间比较存在显著性差异(2=8.19,p<0.05);新生儿窒息发生率羊水正常组5.66%,羊水偏少组19.05%,羊水过少组为37.50%,羊水偏少组、羊水过少组与羊水正常组之间比较存在显著性差异(2=4.58,2=5.58,p<0.05)。结论胎膜早破时残余羊水量的多少与妊娠结局密切相关,羊水残余量越少胎儿的预后相对越会受到影响,并且剖宫产率会明显增加。

THE EFFECT OF PREMATURE RUPTURE OF FETAL MEMBRANES RESIDUAL AMNIOTIC FLUID VOLUME TO PREGNANCY OUTCOME

Objective To explore the effect of different premature rupture of fetal membranes residual amniotic fluid volume to pregnancy outcome. MethodsTo analyze retrospectively the information of 148 preterm premature rupture of membranes pregnancy women,in which 53 cases have normal amniotic fluid volume(normal amniotic fluid volume group),63 cases have less amniotic fluid volum(less amniotic fluid volume group),32 caes have oligohydramnios(oligohydramnios group). ResultsCesarean section rate of oligohydramnios group is 63.13%,which is significantly higher than normal amniotic fluid volume group,whose is 24.53% and less amniotic fluid volume group,whose is 38.10%.It is significantly different between normal amniotic fluid volume group and oligohydramnios group(x2=7.15,p<0.05).Spontaneous vaginal delivery rate of normal amniotic fluid volume group is 69.81%,46.03% in less amniotic fluid volume grou,28.13% in oligohydramnios group.Less amniotic fluid volume group、oligohydramnios group are significantly different from normal amniotic fluid volume group(x2=6.64,x2=13.96,p<0.05).Intrauterine infection rate of normal amniotic fluid volume group is 0.00%,3.18% in less amniotic fluid volume group,12.50% in oligohydramnios group.It is significantly different among the three groups(x2=8.23,p<0.05).Intrauterine fetal distress incidence rate of normal amniotic fluid volume group is 11.32%,20.64% in less amniotic fluid volume group,37.50% in oligohydramnios group.It is significantly different between oligohydramnios group and normal amniotic fluid volume group(x2=8.19,p<0.05).Neonatal asphyxia rate of normal amniotic fluid volume group is 5.66%,19.05% in less amniotic fluid volume group,37.50% in oligohydramnios group.Less amniotic fluid volume group、oligohydramnios group are significantly different from normal amniotic fluid volume group(x2=4.58,x2=5.58,p<0.05). ConclusionResidual amniotic fluid volume number has a close relationship with pregnancy outcome when fetal membrane is early broken.The less Residual amniotic fluid volume to be,the more easily fetal prognosis will be affected relatively.And cesarean section rate will be increased significantly.
