清洁手术围术期抗菌药物使用的干预研究

目的 探讨清洁手术围术期抗菌药物合理应用的有放干预措施.方法 调查某院2011年4月及10月出院的450例清洁手术患者抗菌药物使用情况,其中4月为干预前组(205例),10月为干预后组(245例)；干预措施包括教育性干预、行政性干预和限制性干预.结果 干预后清洁手术的使用率由93.17％下降至11.43％(x2=298.517,P=0.000),其中甲状腺手术抗菌药物使用率仅为3.73％；抗菌药物使用品种由5种下降至1种,用药选择相对合理；且手术部位感染率无明显变化.结论 该院实施的综合干预措施切实可行,有效促进了抗菌药物的合理应用.

Case-control study on intervention in antibiotic application during perioperative period of clean operation

OBJECTIVE To explore the effective intervention measures for the rational application of antibiotics during perioperative period of clean operation.METHODS A total of 450 patients who underwent clean surgery and discharged in Apr and Oct 2011 were selected,then the application of antibiotics was investigated,the patients who discharged in Apr(205 cases) were set as the group before intervention and the patients who discharged in Oct(245 cases) were set as the group after intervention;the intervention measures included the educational intervention,administrative intervention and restrictive intervention.RESULTS The utilization rate of antibiotics decreased from 93.17% to 11.43% after the intervention to the clean operation(χ2=298.517,P=0.000),the utilization rate of antibiotics for the thyroid surgery was 3.73%;the varieties of the antibiotics decreased from 5 to 1,the choice of antibiotics was relatively reasonable;there was no significant change in the incidence of surgical site infections.CONCLUSION The comprehensive intervention measures is feasible and effective in promoting the rational use of antibiotics.
