轻中度高血压患者血尿酸水平调查分析

摘    要:目的 了解来自中国6个城市轻中度高血压患者中血尿酸(serum uric acid,SUA)水平的流行特点及相关因素.方法 对哈尔滨、沈阳、北京、西安、上海和南京6个城市18 ~ 75岁480名轻中度高血压患者进行调查,调查内容包括流行病学问卷、体格及生化检查.结果 共有456例患者纳入最终统计分析,人群尿酸水平平均为296.5 μmol/L,高尿酸血症(hyperuricemia,HUA)患病率为16.5%;多元回归分析表明,男性、高体质指数(body mass index,BMI)、高甘油三酯(triglycerides,TG)、高肌酐(creatinine,Cr)、高尿素氮(blood urea nitrogen,BUN)水平与SUA水平升高,与HUA患病呈正相关;在空腹血糖(fasting blood glucose,FBG)< 7.5 mmol/L下FBG与SUA呈正相关(r =0.21,P<0.001),在FBG≥7.5 mmol/L下FBG与SUA与其呈负相关(r=-0.42,P=0.016).结论 调查的轻中度高血压患者SUA水平偏高,SUA与性别、BMI、TG、Cr、BUN、FBG等因素密切相关.

Abstract:Objective To study the distribution and associated factors of serum uric acid (SUA) levels among mild and moderate hypertensive adults from six cities in China. Methods 480 subjects aged 18 to 75 years were randomly recruited from Harbin, Shenyang, Shanghai, Beijing, Xi' an, and Nanjing. All participants had a questionnaire, physical exam and laboratory testing completed in the study. Results Of the 456 hypertensive patients with complete data in final analysis, the mean SUA level of total population was 296. 5 μmol/L, the prevalence of hyperuricemia (HUA) was 16. 5%. In muhivariable linear or logistic regression models, sex (male versus female), body mass index (BMI), triglyc- erides (TG) , serum ereatinine (Cr) and blood urea nitrogen (BUN) were positively associated with SUA levels and/or hy- peruricemia. Furthermore, we found that fasting blood glucose (FBG) was positively associated with SUA levels when FBG 〈 7.5mmol/L ( r = 0. 21, P 〈 0. 001 ), and negatively associated with SUA levels when FBG ≥ 7. 5mmol/L ( r = - 0. 42, P = 0. 016). Conclusions There were high SUA levels among mild and moderate hypertensive adults from the six cities in China. SUA levels were closely associated with Sex, BMI, TG, Cr, BUN and FBG.
