B族溶血性链球菌感染与胎膜早破、早产、晚期先兆流产的关系及干预

目的:探讨生殖道B族溶血性链球菌(GBS)感染与胎膜早破、早产、晚期先兆流产的关系,并提出相关干预措施,以降低胎膜早破、早产、晚期先兆流产的发生率。方法:以因胎膜早破、早产、晚期先兆流产住院的孕妇236例为研究组,随机抽取相同孕周的正常孕妇236例为对照组,比较两组宫颈粘液GBS阳性率;将门诊筛查宫颈粘液GBS阳性的20～32周孕妇70例随机分为37例治疗组和33例未治疗组,比较抗GBS感染对母婴预后的影响。结果:研究组孕妇宫颈粘液GBS阳性率明显高于对照组孕妇(P<0.01);早期抗GBS感染能降低胎膜早破、早产、晚期先兆流产的发生率。结论:胎膜早破、早产、晚期先兆流产与GBS感染密切相关;对于分泌物GBS筛查阳性的孕妇,早期抗GBS治疗能够改善孕妇及围产儿的预后;将GBS筛查列入围产期检查是必要的。

Treatment and relationship between premature rupture of fetal membrane, premature labor, threatened abortion and group B streptococcus

Objective:To explore the treatment and relationship between premature rupture of fetal membrane, premature labor, threatened abortion and group B streptococcus (GBS).Methods:236 pregnant women diagnosed as premature rupture of fetal membrane, premature labor or threatened abortion were selected as study group while other 236 healthy pregnant women as control group, GBS positive rates were compared; among 486 pregnant women whose gestational periods were during 20 to 32 weeks, 70 women whose GBS screening were positive were divided into treatment group and without treatment group.Results:The rate of GBS infection in study group was higher than that in control group (P<0.01); anti-GBS treatment reduced the morbidity of premature rupture of fetal membrane, premature labor and threatened abortion (P<0.05).Conclusion:Premature rupture of fetal membrane, premature labor and threatened abortion were related to infection of GBS; anti-GBS treatment can improve prognosis of mothers and infants; it is necessary to screen GBS during pregnancy.
