妊娠晚期羊水指数减少对妊娠结局的影响

目的:探讨孕晚期羊水指数减少对妊娠结局以及围生儿情况的影响。方法:选择2011年4月～2012年2月确诊的78例羊水指数减少的孕妇为研究组,随机选取同期93例羊水正常的孕产妇为对照组,比较两组孕妇的并发症发生率、分娩方式及围生儿情况(脐带绕颈、胎儿窘迫、生长受限、新生儿窒息等)。结果:研究组妊娠并发症发生率和围生儿并发症发生率明显高于对照组(P<0.05),研究组剖宫产例数也显著高于对照组(P<0.05)。结论:羊水指数减少不仅是影响妊娠结局的重要因素,也是孕晚期影响胎儿健康的危险信号,临床上应加强进行产前监测,对羊水指数减少的孕妇采取积极的处理和干预,以改善围生儿预后。

Effect of decline of amniotic fluid index during the third trimester of pregnancy on pregnancy outcome

Objective:To explore the effect of decline of amniotic fluid index during the third trimester of pregnancy on pregnancy outcome and perinatal situation.Methods:Seventy-eight pregnant women who were diagnosed as decline of amniotic fluid index definitely from April 2011 to February 2012 were selected as study group,while 93 pregnant women with normal amniotic fluid during the same period were randomly selected as control group;the incidence rates of complications,delivery modes,and perinatal situations(cord around neck,fetal distress,fetal growth restriction,and neonatal asphyxia) in the two groups were compared.Results:The incidence rates of pregnancy and perinatal complications in study group were statistically significantly higher than those in control group(P<0.05),the number of pregnant women adopting cesarean section in study group was statistically significantly higher than that in control group(P<0.05).Conclusion:The decline of amniotic fluid index in not only an important influencing factor of pregnancy outcome,but also a danger signal of fetal health during the third trimester of pregnancy;prenatal monitoring should be enhanced in clinic,the pregnant women with decline of amniotic fluid index should be treated and intervened actively to improve perinatal prognosis.
