心包内全肺及左心房部分切除治疗Ⅲb期肺癌

目的探讨心包内全肺及左心房部分切除治疗Ⅲb期肺癌的手术指征和技术操作要点。方法1990年1月～1998年5月，心包内手术切除Ⅲb期肺癌56例，合并左心房部分切除12例，心包大部切除、心脏裸露8例。结果全组无手术死亡，术后发生并发症5例，发生率8.9%。术后1、3、5年生存率分别为85.2%、31.6%和25%。结论对Ⅲb期肺癌采用扩大手术切除能延长生存期，提高患者生活质量，有临床应用价值。

Interpericardium pneumonectomy and left atrium partial resectio n for

Objective　To explore the operative indications an d the key points of operational technique of interpericardium pneumonectomy and left atrium partial resect on Ⅲb stage lung cancer.Methods　F rom jan,1990 to May,1998,interpericardium pneumonectomy was performed in 56 case s of Ⅲb stage lung cancer,with left atrium partial removed in 12,main part of pericardium removed and the heart uncovered in 8 cases.Results　There was no operative death in this group.The occurrence rate of postoperatio n complication was 8.9%(5/56).The 1,3 and 5 year post-operation survial rate wa s 85.2%,31.6% and 25% respectively.Conclusions　The extensive op eration can prolong the survial period and improve the living guality,so it has clinical significance.
