老年早期乳腺癌患者行乳腺区段切除术后生存分析

目的:评价老年早期乳腺癌患者行乳腺区段切除术后疗效。方法:回顾分析我院2011年1月到2016年4月共65例70岁以上女性早期老年乳腺癌患者的临床病理特征、转移复发及生存资料。患者均接受乳腺区段切除，术后根据情况行内分泌治疗或放疗。结果:中位随访42个月，共有6例患者出现同侧局部复发或合并远处转移，10例死亡，其中3例因乳腺癌转移死亡，7例死于其它疾病。3年累积乳腺癌特异性DFS 91.1%，3年累积OS 90.2%。单因素分析显示Ki67表达>14%（P=0.009）以及ER/PR阳性患者未接受内分泌治疗（P=0.023）是出现转移复发的独立危险因素。结论:对于70岁以上老年早期乳腺癌患者给予乳腺区段切除治疗是一种安全有效的方法。

Survival analysis of patients aged 70 years or older with early breast cancer after lumpectomy treatment

Objective:To evaluate the outcome of patients aged 70 years or older with early breast cancer who received lumpectomy treatment.Methods:The data of 65 early breast cancer patients，who received treatment in Peking University Third Hospital from 2011 to 2016，were collected and analyzed retrospectively.All the cases received lumpectomy without axillary lymph nodes dissection，and then radiotherapy or endocrinotherapy were arranged postoperatively according to the clinical features of patients．Results:The median time of follow-up were 42 months.Six cases got ipsilateral local recurrence with or without distant metastasis.There were 10 deaths，3 of the deaths results from breast cancer.The respective 3-year breast cancer-specific DFS estimates were 91.1%，and 3-year OS estimates were 90.2%.Ki67>14%（P=0.009）and endocrinotherapy missing （P=0.023）in patients with ER/PR positive breast cancer were all correlated with DFS significantly in the univariate analysis.Conclusion:Lumpectomy treatment is safe and feasible for early breast cancer patients aged 70 or older.
