非小细胞肺癌低分割放疗后放射性肺炎的影响因素分析

目的 探讨非小细胞肺癌低分割放疗后放射性肺炎的影响因素.方法 收集120例接受低分割放疗的非小细胞肺癌患者的临床资料,对术后3个月内发生放射性肺炎的危险因素进行Logistic回归分析.结果120例患者中,37例(30.83%)患者发生放射性肺炎,83例(69.17%)患者未发生放射性肺炎.有肺部疾病史、肿瘤体积≥40 cm3、V20≥12%、患肺最大受照剂量≥54 Gy、有化疗史是影响非小细胞肺癌低分割放疗后放射性肺炎的独立危险因素(P﹤0.05).结论 非小细胞肺癌低分割放疗后放射性肺炎的发生是各项指标相互作用的结果,在制定放疗计划时需综合考虑各项指标,及时调整治疗方案以降低放射性肺炎的发生率.

Factors influencing the radiation pneumonitis in non-small cell lung cancer patients after hypofractionated radiation therapy

Objective To investigate the factors influencing the occurrence of radiation pneumonitis in non-small cell lung cancer (NSCLC) patients who underwent hypofractionated radiation therapy. Method 120 NSCLC patients who re-ceived hypofractionated radiation were included in this analysis, and the risk factors of radiation pneumonitis in 3 months after surgery in these patients were analyzed using Logistic regression. Result Of the 120 cases, 37 (30.83%) had radia-tion pneumonitis, while another 83 cases (69.17%) had not. A history of lung disease, tumor volume ≥40 cm3, V20 ≥12%, maximum exposure dose≥54 Gy, prior history of radiotherapy were independent risk factors of developing radia-tion pneumonitis after hypofractionated radiation (P<0.05). Conclusion The occurrence of radiation pneumonitis is the result of the interaction of various indicators, therefore, in the process of establishing radiotherapy plan, we should com-prehensively consider all the indexes and adjust the treatment plan in time to reduce the risk of radiation pneumonitis.
