肝癌患者病灶局部免疫状况研究

目的：评价肝细胞肝癌患者病灶局部免疫状况。方法：在超声引导下经皮进行肝脏穿刺活检，取出的组织标本用特异性抗体免疫组织化学染色，检测病灶区域浸润的T淋巴细胞、自然杀伤细胞、巨噬细胞、B淋巴细胞及T淋巴细胞Fas-L阳性率，测量阳性细胞占单位面积百分比及阳性细胞直径，以了解病灶局部免疫细胞分布情况。结果：肝癌病灶局部有不同程度免疫细胞浸润，主要为T淋巴细胞、自然杀伤细胞和巨噬细胞，部分T淋巴细胞和自然杀伤细胞直径＞10μm，一些巨噬细胞有突起，8．0％ T淋巴细胞Fas-L表达阳性。免疫细胞浸润程度依次为肝癌被膜下＞癌周肝组织＞肝癌组织。结论：肝癌患者病灶局部有免疫活性细胞浸润，但限于癌被膜下及癌周肝组织内，癌组织内免疫活性细胞浸润程度低下。

Study on the Local Immune Status of the Patients with Hepatocellular carcinoma

Objective:To assess the local immune status of the patients with primary hep-atocellular carcinoma (HCC).Methods:The specimens of HCC and adjacent liver tissues were taken through ultrasound-guided percutaneous liver biopsy and the specific monoclonal antibodies were used for detecting the local infiltrating T cells,natural killer cells(NK cell),macrophages,B lymphocytes and the positive rate of Fas-1expression,to assess the distribution of local immune cells of the le sion the percentage of unit area and the diameter of positive cells were measured.Re sults:Some infiltrating T lymphocytes,macrophages and NK cells were detected in the lesion site.The degree of immunocytes infiltration was different from patients to patients.The dimen-sion of a part of lymphocytes and NK cells were larger than10um,some macrophages had anten-nas and8.0%of T lymphocytes expressed Fas-1.The infiltrating degree of immunocytes was higher in the capsule of tumor than that in adjacent tissues and HCC tissues.Conclusion:There is activated immunocyte infiltration in the lesion site of liver,however it is only limited within the capsule and adjacent liver tissue,the degree of activated immunocyte infiltration is low in the tumor tissues.
