摘    要:目的:探讨采用常规腹腔镜手术器械行改良经脐单孔腹腔镜胆囊切除术(laparoscopic cholecystectomy,LC)的应用价值、适应证及操作技术要点。方法:回顾分析2015年1月至2016年12月137例采用常规腹腔镜手术器械行改良经脐单孔LC患者的临床资料。结果:128例顺利完成改良经脐单孔LC,成功率93.43%。9例由于术中操作困难改行两孔或三孔法LC,无一例中转开腹。术后患者均恢复良好,无术后出血、胆漏、继发胆总管结石等并发症发生。结论:采用常规腹腔镜手术器械行改良经脐单孔LC是安全、可行的,与其他经脐单孔LC相比,改良LC在减少手术费用与创伤,增加手术操作的舒适性、安全性及术后美容效果等方面均具有明显优势,值得推广应用。

Abstract:Objective:To discuss the applications,indications and technological key points of improved transumbilical single incision laparoscopic cholecystectomy (TUSILC) by conventional laparoscopic surgical instruments.Methods:The clinical data of 137 patients who underwent improved TUSILC by conventional instruments from Jan.2015 to Dec.2016 were analyzed retrospectively.Results:There were 128 of 137 cases (93.43%) finished improved TUSILC by conventional instruments.9 cases were converted to two-port or three-port laparoscopic cholecystectomy due to intraoperative difficulty,and no one converted to laparotomy.All patients recovered well and no complications like hemorrhage,bile leakage or secondary choledocholithiasis occurred.Conclusions:Improved TUSILC by conventional laparoscopic surgical instruments is safe and feasible.Compared to the other TUSILC,this operation has obvious advantages in cutting down surgical costs,reducing surgical damage and improving the comfort,security and cosmetic results of operation.It deserves generalization and application.
