传动直丝弓矫治技术对成人高角开牙合患者矫治疗效的探讨

摘    要:目的:探究传动直丝弓矫治技术对成人高角开牙合病例的临床疗效及矫治体会。方法:对34例成人高角开牙合患者采用传动直丝弓技术进行矫治,所有患者均拔除4颗前磨牙;根据患者开牙合的严重程度分为两组:中度组(3 mm≤前牙开牙合≤5 mm)和重度组(5 mm<前牙开牙合≤8 mm),每组各17例患者。中度组单独使用传动直丝弓矫治技术进行治疗,重度组在传动技术矫治第二阶段辅以后牙区颊腭侧种植体支抗压低上颌磨牙。治疗前后均拍摄X线头颅侧位片,比较两组患者矫治前后头影测量指标的变化。结果:34例患者中31例顺利完成治疗,牙性指标如U1/SN、L1/MP、U1-NA、U1/L1等,中度组改变较重度组显著;骨性指标NP/FH、SN/MP、ODI、APDI、ANS-Me/N-Me、S-Go/N-Me等和软组织指标NsPos/FH、NLA、LL-SnPos等,重度组改变较中度组显著;特别值得关注的是牙性指标U6-PP、L6-MP在中度组没有明显改变,但在重度组U6-PP明显改变。结论:传动直丝弓技术矫治高角开牙合患者,可快速改善覆牙合覆盖及面部软硬组织突度,具有疗程短见效快稳定性好的优点;对重度骨性开牙合患者,配合后牙区种植体支抗能有效降低垂直高度,下颌发生逆时针旋转,颌骨及面部软组织侧貌改善更为显著。
