周围型小肺癌：瘤—肺界面的HRCT与病理对照研究

本文对病理证实的周围型小肺癌３２例的瘤—肺界面进行了ＨＲＣＴ与病理的前瞻性对照研究。术前有ＨＲＣＴ扫描，术后标本充气后ＨＲＣＴ扫描，然后行Ｈｅｉｔｓｍａｎ法固定４８－７２，小时，标本制成组织学切片。对照观察术前．术后标本ＨＲＣＴ及组织学切片，结果表明ＨＲＣＴ能精确反映瘤—肺界面的大体病理改变。周围型小肺癌ＨＲＣＴ常表现为（１）分叶，（２）毛刺，（３）边缘模糊或部分边缘模糊，（４）纵隔窗上边缘锐利。作者初步认为ＨＲＣＴ对周围型小肺癌定性诊断十分重要。

HRCT-Pathologic Correlation of the Lung-Tumor Interface in Small Peripheral Lung Cancer

A prospective study of HRCT-Patholopical corrlation about the lung-tumor interface in 32 cases of small peripheral lung cancer(SPLC) waa reported. After being filled with all, surgically resected fresh lung specimens were scaned with HRCT technique, then fixed by Heitzman method. About 48-72 hoars later, the examination of gross morphol and histology were done. The appearances of the lung-tumor interface on preoperative HRCT and fresh specimen HRCT images were correlated with that of their patholgy. The results showed that HRCT could accurately reflect the macrosoopic changes throughout the lung-tumor interface. The HRCT appearaces of SPLC could be presented as (1)notch or lobulation,(2) Spiculation, (3 )shaggy and cloudy margin, and (4)sharp and smooth contour. Good correlation between HRCT imagpathology supported the conclusion that HRCT was very important and favorable for and accurate diagnosis of SPLC.
