脑卒中病人延续护理方案的建立和应用效果临床评价

目的]建立脑卒中病人延续性护理方案,分析其护理效果。方法]选择脑卒中病人82例,按照随机数字表法将病人分为研究组和对照组各41例。对照组病人给予常规出院护理,研究组病人给予脑卒中延续性护理。观察两组病人出院前、出院后3个月Barthel指数量表变化、生活质量(QOL)量表变化、依从性评分及护理满意度。结果]出院前两组病人Barthel指数评分比较差异无统计学意义(P0.05),出院后3个月研究组病人Barthel指数评分均显著高于对照组(P0.05)。出院后3个月研究组病人QOL各项评分和总分均显著高于对照组(P0.05)。研究组病人按时服药、按时锻炼、按时复诊和合理饮食比例均高于对照组,两组比较差异无统计学意义(P0.05)。研究组病人对护理工作的总满意率显著高于对照组(P0.05)。结论]延续护理可以有效地提高脑卒中病人治疗依从性和满意度,提高脑卒中病人的护理效果。

Clinical evaluation on establishment and application effect of continuity of care for patients with stroke

Objective:To establish the continuity of care program for patients with cerebral apoplexy,and analyze the nursing effect.Methods:A total of 82 patients with cerebral apoplexy were selected and randomly divided in-to study group and control group based random number table,41 cases in each.Patients in control group were given routine discharge care,the patients in study group were given the continuity of stroke care.The changes of Barthel index scale,quality of life (QOL)scale,compliance score and nursing satisfaction in both groups were observed before discharge and 3 months after discharge.Results:There was no statistically significant difference in Barthel index scores in patients between both groups before discharge(P >0.05).3 months after discharge, the Barthel index score in study group was significantly higher than that in control group (P <0.05),Barthel in-dex scores of patients in study group were significantly higher than those in control group (P <0.05).3 months after discharge,the QOL scores and total score of the patients in study group were significantly higher than those in control group (P <0.05).The ratios of medication on time,exercise on time,timely referral and reason-able diet in patients in study group were higher than that in control group,but there was no statistically signifi-cant difference between both groups (P > 0.05).The total satisfaction rate of the patients in study group was significantly higher than that in control group (P <0.05 ).Conclusion:Continuity of care could effectively im-prove the treatment compliance and satisfaction of patients with cerebral apoplexy,and improve the nursing effect of patients with cerebral apoplexy.
