肝豆状核变性患者角膜K-F环分析

目的回顾性分析肝豆状核变性(WD)患者K-F环同临床分型、病程和性别的相关性。方法将符合纳入标准的924例WD患者分为肝型、脑型、脑-内脏型。K-F环检测有经验丰富的医师在暗室中利用裂隙灯检查。结果 WD患者肝型组K-F环分级较脑型、脑-内脏型存在明显差异性(P=0.00);K-F环阴性组与K-F环2级、3级、4级之间比较均有差异性(P=0.00);K-F环阳性组之间比较均无统计学差异。K-F环阴性患者平均病程(14.9±27.3)个月]和阳性患者平均病程(32.0±50.5)个月]具有统计学差异(P=0.00);另外K-F环阴性组WD患者平均病程与K-F环2、3、4级WD患者平均病程比较有统计学差异(P值分别为0.00、0.03、0.00);而K-F环阳性(即1、2、3、4级)WD患者之间平均病程比较无统计学差异;K-F环同性别比较无统计学差异。结论 K-F环同临床分型、病程具有密切相关性,K-F环的分级同Goldstein分级(病情的轻重)具有一定的相关性,但未发现完全的正相关性,即随着K-F分级的增加患者的Goldstein分级也随之增加;且未发现K-F环同性别具有相关性。

Analysis on Kayser-Fleischer ring in hepatolenticular degeneration

ObjectiveTo retrospectively observe the relation between the corneal Kayser- Fleischer（K-F） ring in Wilson＇s disease and their clinical classification types, the course of disease, the gender and the grade of Goldstein.Methods 924 patients which meet the entry criteria were classified as the group of liver type, brain type and brain-visceral type. Kayser-Fleischer ring was checked by empirical doctor on slit-lamp examination in darkroom.Results The K-F ring＇s grade in liver type of WD was significantly lower than brain and brain-visceral type （P=0.00）. There had significant between K-F ring negative group and grade 2, 3, 4 of K-F ring （P=0.00）, but there had not significant between group of K-F ring positive. The average of duration in K-F ring negative group was significantly lower than K-F ring positive which grade in 2, 3, 4. The duration had not significant in group of K-F ring positive. The ration of K-F ring positive and negative also had not significant in gender.Conclusions K-F ring have close relation to clinical classification types and the duration of Wilson＇s disease. The K-F ring grade has part relation to the grade of Goldstein scale, but not find positive correlation thoroughly. And we also have not find correlation between K-F ring and the gender in Wilson＇s disease.
