大视野高空间分辨力三维增强磁共振头颈静脉成像中的对比剂注入方法

摘    要:目的 探讨采用大视野高分辨力三维对比增强磁共振静脉成像(3D CE MRV)进行头颈静脉成像时的对比剂注入方法。方法 对37例无神经系统症状体征、影像学无异常发现的门诊患者及2名健康志愿者行大视野高分辨力头颈静脉3D CE MRV,根据对比剂注入方法分为单相团注组(A组)和双相团注组(B组),分析两组图像整体质量、伪影情况、动脉干扰评分差异和动静脉信号比值、背景组织与静脉信号强度比值,采用独立样本秩和检验和独立样本t检验进行统计分析。结果 两组3D CE MRV成像的图像总体质量评分、伪影情况评分、动脉信号干扰评分、背景组织与静脉信号强度比值、颅内动静脉信号比值差异均无统计学意义(P均>0.05),而B组颈部动静脉信号比值高于A组(P<0.05)。结论 3.0T场强下3D CE MRV头颈静脉联合成像中,应用单相团注单倍(0.1 mmol/kg体质量)剂量对比剂即可获得高质量静脉血管增强影像,完整显示头颈静脉系统。

Abstract:Objective To explore administration method of contrast agent for large field of view high-spatial-resolution 3D contrast-enhanced magnetic resonance venography (3D CE MRV) of cerebral venous sinus and jugular veins. Methods Totally 39 subjects were examined with large field of view high-spatial-resolution 3D CE MRV, and were randomly assigned into single phase bolus chase technique group (group A) or dual phase bolus chase technique group (group B). Image quality, artifacts, arterial contamination, signal intensity of background tissue to signal intensity of vein ratio and signal intensity of artery to signal intensity of vein ratio were evaluated and compared between the two groups by using Mann-Whitney U and independent-sample t test. Results There was no significant difference of image quality, image artifacts, arterial contamination, signal intensity of background tissue to signal intensity of vein ratio or cerebral signal intensity of artery to signal intensity of vein ratio between the two groups (all P>0.05), while jugular signal intensity of artery to signal intensity of vein ratio of group B was higher than that group A (P<0.05). Conclusion Under 3.0T, large field of view high-spatial-resolution 3D CE MRV can be performed with single dose (0.1 mmol/kg) contrast agent and single phase bolus chase technique to obtain high-quality images of the whole of intracranial and neck venous vessels.
