氯诺昔康超前镇痛对骨科术后芬太尼镇痛效果的影响

摘    要:目的观察氯诺昔康用于骨科手术超前镇痛对芬太尼术后镇痛作用的影响。方法60例ASAⅠ-Ⅱ级择期行骨科手术的患者,按随机数字表随机分为L组与F组,每组30例。L组术前10min静脉滴注氯诺昔康0.2mg/kg,F组术前静脉滴注等量的生理盐水,分别记录术后开始镇痛后2,4,6,812,24h的视觉模拟评分(VAS)、24h芬太尼用量及不良反应。并在镇痛结束后统计患者对镇痛治疗的总体满意度。结果术后2,4,6,8,12,24hVASL组均低于F组(P〈0.05),术后24hL组的芬太尼用量(0.467±0.058)]mg低于F组的(0.647±0.043)]mg(P〈0.05),两组的不良反应除了嗜睡和呕吐外差异无显著意义(P〉0.05)。镇痛结束后,患者对镇痛治疗总体满意度L组与F组在Ⅱ级和Ⅲ级差异有显著意义(P〈0.05)。结论氯诺昔康在用于骨科手术前超前应用能减少术后芬太尼镇痛用量,减轻一些不良反应的发生并能提高镇痛质量.

Abstract:Objective To study the regulation of HCV NS3 protein on the expression of survivin prorein in QSG7701 cell and the interacion with apoptosis for investigating carcinogenesis mechanism of HCV NS3 protein in human liver cell. Methods Apoptosis induced by serum starvation or not, the expressions of survivin protein are detected by Western Blot and immunocytochemistry, the apoptosis indexes are examined by TUNEL.Results HCV NS3 protein activates the expression of survivin in QSG7701 cell. After apoptosis induced by serum starvation, the expressions of survivin are down-regulated. HCV NS3 protein also activates the expression of survivin which exhibits the tendency of time and concentration in QSG7701 cell. At the same time, HCV NS3 protein inhibits apoptosis induced by serum deprivation in QSG7701 cell. Conclusion HCV NS3 protein inhibits apoptosis by up-regulating the expression of survivin protein in QSG7701 cell, contributes to malignant transformation of hepatocytes and hepatocellular carcinogenesis related HCV.
