应用超声心肌做功指数评价早期慢性肾衰患者左室功能

目的探讨超声心肌做功指数(Tei指数)在评价早期慢性肾衰患者左室功能中的价值。方法连续入选经临床及实验室检查明确诊断为慢性肾病的患者60例,分为早期肾功能不全组(24例)和中晚期肾功能不全组(36例);同期选取正常对照组34例进行对照。常规超声测量左室结构和功能,并应用组织多普勒(DTI)计算左室Tei指数。结果早期肾功能不全组常规超声所测左室结构与收缩功能与对照组比较差异均无统计学意义(均P﹥0.05);早期肾功能不全组和中晚期功能不全组Tei指数均高于对照组(P﹤0.05);Tei指数与内生肌酐清除率(Ccr)呈显著相关(P﹤0.05)。结论 Tei指数早期检出肾功能不全患者左室功能障碍。

The application of myocardial performance index in evaluating the left ventricular function of patients with early chronic renal failure

Objective to investigate the application of myocardial performance index（Tei index）in evaluating the left ventricular function of patients with early chronic renal failure.Methods 60 patients with chronic renal failure were divided into early renal failure group（24 cases）and advanced renal failure group（36 cases）according to the level of the creatinine clearance rate（Ccr）,and 34 healthy physical examinees were selected as control group.Structure and function of left ventricles were measured by regular ultrasonography.The Tei index was measured by tissue Doppler.Results There were on differences in structure and function indicators of left ventricles between the early renal failure group and control group（P﹥0.05）.The Tei index of control group、early renal failure group,advanced renal failure were 0.42 ±0.09,0.52 ±0.11,0.58 ±0.12;the tei index of early renal failure group was higher than that of control groups（P﹤0.05）.There were significant correlation between Tei index and Ccr level（P﹤0.05）.Conclusion Tei index can effectively evaluate left ventricular function in patients with early chronic renal failure.
