骨髓增殖性肿瘤患者NGAL、VEGF、PC及PS表达水平及意义研究

目的观察骨髓增殖性肿瘤(MPN)患者中性粒细胞明胶酶相关脂质运载蛋白(NGAL)、血管内皮生长因子(VEGF)、血浆蛋白C(PC)及血浆蛋白S(PS)表达水平,并分析其临床意义。方法回顾性选取2019年1月至2021年8月在沧州市中心医院初诊MPN患者40例、MPN治疗后患者40例,以及健康体检者40例为研究对象,分别作为初诊组、治疗后组、对照组。观察3组研究对象、不同临床特征MPN患者的NGAL、VEGF、PC及PS表达水平,分析影响MPN患者的NGAL、VEGF、PC及PS表达水平的因素。结果初诊组、治疗后组的NGAL(18.34±2.11)、(10.49±1.87)ng/mL]、VEGF(239.29±10.45)、(151.34±9.13)pg/mL]水平均显著高于对照组(2.37±0.45)ng/mL、(78.66±8.35)pg/mL],初诊组、治疗后组的PC(1794.35±142.49)、(2918.21±85.35)ng/mL]、PS(43424.64±102.41)、(63467.47±125.48)ng/mL]水平均显著低于对照组(3265.46±72.98)ng/mL、(94253.32±178.23)ng/mL],差异均有统计学意义(P<0.05)。合并血栓、有脾大、血红蛋白<100g/L、血小板<10×10⁹/L的MPN患者的NGAL、VEGF水平较高,PC、PS水平较低,差异均有统计学意义(P<0.05)。多重线性回归分析结果显示,发现有无血栓和血红蛋白水平均是影响MPN患者NGAL、VEGF、PC及PS水平的因素(P<0.05)。结论MPN患者NGAL、VEGF水平较高,PC、PS水平较低,治疗后会改善,且与患者是否合并血栓、血红蛋白水平相关。

Study on the expression levels and significance of NGAL,VEGF,PC and PS in patients with myeloproliferative neoplasms

Objective To observe the expression levels of neutrophil gelatinase-associated lipocalin(NGAL),vascular endothelial growth factor(VEGF),plasma protein C(PC)and plasma protein S(PS)in patients with myeloproliferative neoplasms(MPN),and analyze its clinical significance.Methods Forty newly diagnosed MPN patients,40 patients after MPN treatment,and 40 healthy patients who were admitted to Cangzhou Central Hospital from January 2019 to August 2021 were retrospectively selected as research subjects.They were used as the newly diagnosed group,the post-treatment group,and the control group.The expression levels of NGAL,VEGF,PC and PS in the three groups of patients and patients with MPN with different clinicopathological characteristics were observed,and the factors affecting the expression levels of NGAL,VEGF,PC and PS in patients with MPN were analyzed.Results The levels of NGAL(18.34±2.11),(10.49±1.87)ng/mL]and VEGF(239.29±10.45),(151.34±9.13)pg/mL]in the newly diagnosed group and the post-treatment were significantly higher than those of the control group(2.37±0.45)ng/mL,(78.66±8.35)pg/mL],and the levels of PC(1794.35±142.49),(2918.21±85.35)ng/mL]and PS(43424.64±102.41),(63467.47±125.48)ng/mL]in the newly diagnosed group and the post-treatment were significantly lower than those in the control group(3265.46±72.98)ng/mL,(94253.32±178.23)ng/mL],and the differences were statistically significant(P<0.05).Patients with myeloproliferative tumors with thrombosis,splenomegaly,hemoglobin<100 g/L,platelets<10×10⁹/L had higher levels of NGAL and VEGF,and lower levels of PC and PS,and the differences were statistically significant(P<0.05).Multiple linearity Regression analysis results showed that the presence or absence of thrombus and hemoglobin levels are factors that affect the levels of NGAL,VEGF,PC and PS in patients with myeloproliferative tumors(P<0.05).Conclusion The levels of NGAL and VEGF in patients with MPN are higher,and the levels of PC and PS are lower,which will be improved after treatment,and it is related to whether the patient has thrombosis and hemoglobin level.
