摘    要:目的:探讨关节腔积液有形成分分析在骨关节病中的诊断价值。方法:收集我院2017—2018年送检的660例关节腔积液,用普通光学显微镜及偏光显微镜观察关节腔积液有形成分。结果:痛风患者尿酸钠结晶或尿酸钠吞噬细胞阳性率为73.56%(192/261)。660例关节腔积液中,赖特细胞检出率为4.85%(32/660);脂肪滴检出率为2.73%(18/660);焦磷酸钙结晶和噬菌细胞检出率均为1.21%(8/660);狼疮细胞和橙色血质结晶检出率均为0.45%(3/660)。结论:关节腔积液有形成分分析在骨关节疾病的诊断和鉴别诊断中能提供重要线索和有力证据,帮助临床医生合理用药,使广大关节骨病患者早治疗早康复。

Clinical significance of examination of formative components in joint effusionYANG Qinglan , Rao Huachun ,Wang Jiawen ,GUO Qingxin(Department of Clinical Laboratory of Quanzhouorthopedic-traumatological hospital,Quanzhou36200,China)
Abstract:Objective To explore the significance of the formation of joint fluid in the joint cavity for the diagnosis of bone and joint diseases. Method Through retrospective analysis of 660 cases of joint cavity fluid in our hospital from 2017 to 2018, the common formation of joint cavity fluid was observed by ordinary light microscope and polarized light microscope. ResultAmong 261 patients with gout, 192 of 660 patients with joint effusion were diagnosed as urate crystals or urate phagocytes. The positive rate was 73.56% Calcium pyrophosphate crystals were detected in 8 cases, lupus cells in 3 cases, Wright cells in 32 cases, bacteria or bacteriophages in 8 cases, fat droplets in 18 cases, orange blood crystals in 3 cases. Conclusion The examination of the constituents of joint effusion can provide important clues and strong evidence for the early diagnosis of joint diseases reasonably, quickly and accurately. Urate crystals or calcium pyrophosphate crystals can be found in joint effusion as an important basis for the diagnosis of crystalline arthritis. Orange blood crystals found in joint effusion can be used to indicate obsolete hemorrhage. Persistent hemorrhage can be considered if accompanied by intact red blood cells. The presence of phagocytic bacteria in joint effusion can provide important clues for the diagnosis of bone and joint infections.
