T2DM合并微血管病变患者凝血四项与C肽、IR水平的关系

目的分析2型糖尿病(T2DM)合并微血管病变患者C肽、胰岛素抵抗(IR)水平与凝血四项凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、凝血酶时间(TT)及纤维蛋白原(Fib)]的关系。方法选取100例T2DM合并微血管病变患者(观察组)及100例T2DM未合并微血管病变患者(对照组)。检测所有研究对象PT、APTT、TT、Fib与C肽、IR水平。采用Pearson相关性分析评价凝血四项与C肽、IR水平的相关性。结果观察组PT、APTT、TT及C肽水平显著低于对照组(P<0.001),Fib水平及胰岛素抵抗指数(HOMA-IR)显著高于对照组(P<0.001)。C肽水平与PT和APTT呈正相关(r值分别为0.302、0.224,P<0.05),IR水平与PT和APTT呈负相关(r值分别为-0.413、-0.253,P<0.05)。结论 T2DM合并微血管病变患者凝血功能障碍与C肽水平下降和IR水平升高有关。

Relationship between four coagulation factors and C peptide and IR in T2DM patients with microangiopathy

Objective To analyze the relationship between four coagulation factorsprothrombin time(PT),activated partial thromboplastin time(APTT),thrombin time(TT)and fibrinogen(Fib)]and C peptide and insulin resistance(IR)in type 2 diabetes mellitus(T2DM)patients with microangiopathy.Methods A total of 100 T2DM patients with microangiopathy were enrolled as observation group,and 100 T2DM patients without microangiopathy were enrolled as control group.PT,APTT,TT,Fib,C peptide and IR levels were determined.Pearson correlation analysis was used to analyze the correlation between four coagulation factors and C peptide and IR levels.Results The levels of PT,APTT,TT and C peptide in observation group were lower than those in control group(P<0.001),and the level of Fib and homeostasis mode assessment for insulin resistance(HOMA-IR)in observation group were higher than those in control group(P<0.001).C peptide was positively correlated with PT and APTT(r=0.302 and 0.224,P<0.05),and IR was negatively correlated with PT and APTT(r=-0.413 and-0.253,P<0.05).Conclusions The coagulation dysfunction of T2DM patients with microangiopathy is related to the decreasing of C peptide and the increasing of IR.
