喹硫平联合无抽搐电休克治疗首发精神分裂症疗效观察

目的：探讨喹硫平联合无抽搐电休克治疗首发精神分裂症患者的临床疗效及安全性。方法将72例首发精神分裂症患者按入院单双号分为两组，均予以富马酸喹硫平治疗，观察组在此基础上联合无抽搐电休克治疗，观察8周。采用阳性与阴性症状量表评定临床疗效，副反应量表评定不良反应。结果观察组总有效率为94.4%，对照组为58.3%，观察组显著高于对照组(P <0.05)。治疗1周末起，两组阳性与阴性症状量表总分均较治疗前显著降低(P <0.01)，观察组显著低于对照组(P <0.01)。观察组不良反应主要表现为记忆障碍、一过性头疼、头晕等，治疗1周后可缓解；对照组主要表现为锥体外系反应，便秘、口干、体质量增加，且发生率显著高于观察组(P <0.05)。结论喹硫平联合无抽搐电休克治疗首发精神分裂症疗效显著，起效快，安全性高，优于单用喹硫平治疗。

Curative effect of quetiapine combined with MECT in the treatment of the first-episode schizophrenia

Objective To explore the efficacy and safety of quetiapine combined with modified electroconvulsive therapy (MECT)in the treatment of the first-ep-isode schizophrenia.Methods Seventy-two first-episode schizophrenics were as-signed to two groups according to odd and even numbers of admission,both groups were treated with que-tiapine fumarate,and on this basis observation was plus MECT for 8 weeks.Efficacies were assessed with the Postive and Negative Syndrome Scale (PANSS)and adverse reactions with the Treatment Emergent Symptom Scale (TESS).Results Total effective rate was respectively 94.4% in observation and 58.3%in control group,the former significantly higher than the latter (P <0.05).Since the end of the 1 st week the total scores of the PANSS of both groups lowered more significantly compared with pretreatment (P <0.01),that was significantly lower in observation than in control group (P <0.01 ).Adverse reactions were mainly dysmnesia,transient headache,dizziness and so on and could relieve after l week treatment in observation group;those were extrapyramidal side effects,constipation,dry month and weight gain in con-trol group,their incidence was significantly higher (P <0.05).Conclusion Quetiapine plus MECT has an evident effect and higher safety and takes effect more rapidly compared with single quetiapine in the treat-ment of first-episode schizophrenia.
