草酸艾司西酞普兰联合早期主动运动康复训练治疗急性缺血性脑卒中对照研究

摘    要:目的探讨草酸艾司西酞普兰联合早期主动运动康复训练治疗急性缺血性脑卒中的临床疗效及安全性.方法将200例急性缺血性脑卒中患者按随机数字表法分为两组,每组100例.两组均予以常规治疗+草酸艾司西酞普兰治疗,研究组在此基础上联合早期主动运动康复训练治疗,观察8周(2个疗程).比较两组临床疗效及不良反应发生率,于治疗前后采用Barthel指数评定日常活动能力,美国国立卫生研究院卒中量表评定疾病严重程度,简易智力状态检查量表评定认知功能,并检测血清C反应蛋白、去甲肾上腺素、5-羟色胺水平.结果研究组总有效率显著高于对照组(P<0.01),不良反应发生率显著低于对照组(P<0.05).治疗前两组各项指标比较差异无统计学意义(P>0.05).治疗后,两组Barthel指数、简易智力状态检查量表评分及血清去甲肾上腺素、5-羟色胺水平均较治疗前显著升高(P<0.01),研究组显著高于对照组(P<0.01);两组美国国立卫生研究院卒中量表评分及血清C反应蛋白水平较治疗前显著降低(P<0.01),研究组显著低于对照组(P<0.01).结论草酸艾司西酞普兰联合早期主动运动康复训练治疗急性缺血性脑卒中临床疗效显著,有利于患者大脑神经递质的调节,促进认知功能恢复,降低神经功能缺损程度,增强患者日常生活能力,安全性较高.

Abstract:Objective To investigate the clinical efficacy and safety of escitalopram oxalate combined with early active exercise rehabilitation training in the treatment of acute ischemic stroke.Methods 200 patients with acute ischemic stroke were enrolled and randomly divided into two groups,each with 100 cases.The control group received routine treatment combined with escitalopram oxalate,while the study group received routine treatment combined with escitalopram oxalate and early active exercise rehabilitation training,observed for 8 weeks(2 courses).Compare the clinical efficacy and the incidence of adverse reactions between the two groups.Before and after treatment,BI was used to assess daily activities,NIHSS was used to assess disease severity,MMSE was used to assess cognitive function,and serum CRP,NE,and 5-HT levels were measured.Results The total effective rate of the study group was significantly higher than that of the control group(P<0.01),and the incidence of adverse reactions was significantly lower than that of the control group(P<0.05).There was no significant difference in the indexes of the two groups before treatment(P>0.05).After treatment,the BI,MMSE,NE scores and 5-HT level were increased in both groups(P<0.01),and were higher in the study group than those in the control group(P<0.01).The NIHSS score and serum C-reactive protein level of the two groups were significantly lower than before treatment(P<0.01),and the study group was significantly lower than the control group(P<0.01).Conclusions Escitalopram oxalate combined with early active exercise rehabilitation training in the treatment of acute ischemic stroke can effectively and safely regulate the balance of brain neurotransmitters,promote the recovery of cognitive function,reduce the degree of neurological deficits,enhance the daily living ability of patients,and have higher safety.
