不同透析膜对维持性血液透析患者血清氨基酸谱的影响

摘    要:目的研究不同透析膜对维持性血液透析患者血清氨基酸谱的影响及其透析液中氨基酸的丢失。方法用高效液相色谱仪分别测定采用聚砜膜(A组)、铜仿膜(B组)、复用铜仿膜(C组)的3组患者透析前、透析后血清氨基酸谱及其透析液中氨基酸的丢失量。结果A组透析后血清谷氨酸、谷氨酰胺、丝氨酸、半胱氨酸、脯氨酸、酪氨酸水平降低(P<0.05);B组血透析前后血清氨基酸谱无明显变化;C组透析后血清天冬氨酸、苏氨酸水平降低(P<0.05);A组和B组患者透析液中丢失的氨基酸差异无显著性,C组患者透析液中缬氨酸、亮氨酸、异亮氨酸、丝氨酸、酪氨酸的丢失明显小于A、B两组(P<0.05)结论不同透析膜对患者血清氨基酸谱的影响是不同的,取决于膜的物理化学特性和生物相容性。

Abstract:Objective To investigate the influence of different membranes on the serumamino acid patterns of hemodialysis patients. Methods Thirty maintenance hemodialysispatients were equaly divided into polysulphone,cuprophan and reuse cuprophan group. Pre- andpost-dialysis serum amino acid patterns of patients and amino acid in spent dialysate weredetected by high performance liquid chromatography respectively. Results The serum aminoacid patterns of patients in the three groups were no difference before dialysis. However,the levels of serum glutamic acid, glutamine, serine, cysteine,proline and tyrosine inpolysulphone group, meanwhile serum aspartic acid and threonine in reuse cuprophan groupdecreased after dialysis (P<0.05), and no significant change of serum amino acid patternwas noted in cuprophan group. The losses of valine,leucine, isoleucine, serine and tyrosinewere lower with reuse cuprophan than with cuprophan and polysulphone. Conclusions Thephysiochemical characteristics and biocompatibility of dialysis membrane influence the serumamino acid patterns of patients.
