老年髓系抗原阳性急性淋巴细胞白血病1例报告并文献分析

目的：提高对老年髓系抗原阳性急性淋巴细胞白血病（My＋ALL）的认识。方法：报告1例老年髓系抗原阳性My＋ALL患者的临床资料和实验室检查特征、治疗经过、预后并结合文献复习讨论。结果：患者骨髓原幼淋巴细胞占42．5％，CD7＋细胞占86．99％，cCD3’细胞占77．24％，CD33＋占91．87％，CD13＋占11．80％，MPO，CD117，CD22，CD19，CD20，CD34均阴性，给予长春新碱＋柔红霉素＋泼尼松（VDP方案）化疗1周期，未缓解，之后给予长春新碱＋泼尼松＋阿克拉霉素＋阿糖胞苷（VP＋AA方案）化疗2周期达缓解。结论：老年髓系My＋ALL恶性程度高，对常规化疗方案治疗无效，需给予兼顾髓系和淋系的化疗方案为宜。

Myeloid antigen-positive acute lymphoblastic leukemia in the elderly

Objective：To enhance the knowledge of myeloid antigen-positive My＋ALL in the elderly. Methods：To report a case of myeloid antigen-positive My＋ALL in the elderly,including clinical features,treatment and prognosis and the related literature was reviewed. Results ..Myeloid antigen-positive ALL was diagnosed by 42.5 % prolympho- cyte in bone marrow, 86. 99% expressed CD7＋, 77. 24% cCD3＋, 91. 87% CD33, tl. 80% CD13＋but none MPO, CDl17,CD22,CD19,CD20 and CD34. VDP regimen were administered with no effect, VP＋AA regimen achieved complete remission after two periods. Conclusion .. Myeloid antigen-positive ALL in the elderly has a fulminant course with poor response to chemontherapy. The chemotherapy regimens were for ALL.
