2型糖尿病患者体脂指标与大血管并发症相关性分析与护理干预措施

摘    要:目的分析2型糖尿病患者体脂指标与大血管并发症的相关性并探讨相应的护理干预措施。方法选取2017年6月至2019年6月我院收治的80例2型糖尿病住院患者为研究对象,其中有41例患者合并大血管相关并发症作为合并组,52例未合并大血管相关并发症的2型糖尿病患者为未合并组;收集两组的体质指数(BMI)、空腹血糖(FBG)、空腹胰岛素(FINS)、糖化血红蛋白(HbAlc)、甘油三酯(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)和血浆纤维醇原激活物抑制物1(PAI-1)。结果合并组腰臀比及甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)、总胆固醇(TC)、空腹血糖(TG)、糖化血红蛋白(Hb Alc)及PAI-1均高于对照组,而高密度脂蛋白胆固醇(HDL-C)低于未合并组(P<0.05);PAI-1与FBG,TG,TC,HbAlc,HDL-C呈正相关性,但与HDL-C呈负相关(P<0.05)。结论2型糖尿病内脏脂肪性肥胖可直接影响胰岛素抵抗水平,成为2型糖尿病引发糖尿病血管并发症的独立危险因素,因此,护理人员应强化血脂和血糖水平拟定相应运动和饮食干预。

Abstract:Objective To analyze the relationship between body fat index and macrovascular complications of patients with type 2 diabetes and explore the corresponding nursing intervention measures.Methods We selected 80 cases of in-patients with type 2 diabetes admitted to our hospital from June 2017 to June 2019 as the sutdy subjects.Among them,41 cases of patients were complicated with macrovascular-related complications as a combined group,and 52 cases of patients who were not combined with macrovascular-related complications at the same time were selected as a uncombined group.The body mass index(BMI),fasting blood glucose(FBG),fasting insulin(FINS),glycated hemoglobin(HbA1c),triglyceride(TG),total cholesterol(TC),high density lipoprotein cholesterol(HDL-C),low density lipoprotein cholesterol(LDL-C),and plasma inositol activator inhibitors1(PAI-1)were collected from both groups.Results The waist-hip ratio and TG,LDL-C,TC,TG,HbAlc and PAI-1 of the combined group were higher than those of the control group,while HDL-C was lower than that of the uncombined group(P<0.05).The PAI-1 and FBG,TG,TC,HbAlc HDL-C was positively correlated,but negatively correlated with HDL-C(P<0.05).Conclusion Visceral fat obesity in type 2 diabetes could directly affect resistance level of insulin and become an independent risk factor for vascular complications of diabetes caused by type 2 diabetes.Therefore,nursing staff should strengthen blood lipid and blood glucose levels and formulate corresponding exercise and diet interventions.
