摘    要:目的:探讨正常人群不同年龄的心肌袖和肺静脉壁的形态学特征,为房颤的发生及其消融治疗术提供形态学依据。方法:观察不同年龄组心脏的肺静脉周径,并用不同的方法进行切片染色、计算机软件对肌袖周径和面积进行测量比较。结果:肺静脉的周径、肌袖的面积在不同年龄有显著差异,组内各静脉间则无。肌袖的肌纤维以不同的走行方向渐变地从左心房延续至肺门。静脉壁和肌袖面积在中年组和老年组内有显著差异。在老年组内可见肌袖多有纤维化改变、弹力纤维减少和静脉壁较薄,中青年组相反。结论:正常肺静脉的心房-静脉交界处的周径、肌袖和组织构造会随着年龄的改变而变化,这种改变可能与房颤发生的病理学机制及与其消融治疗术有关。

Abstract:Objective:To investigate the morphological characteristics of myocardial sleeve and pulmonary vein wall in Chinese of different ages,so as to provide basis for development of atrial fibrillation(AF)and its ablation techniques.Methods:The circumference of pulmonary veins of normal heart were examined,and the circumference and area of myocardial sleeve stained with different methods beforehand were examined in old, middle-aged and young Chinese.Results:The circumference of pulmonary venous presented significant difference among 3 groups,but no significant difference was found between different venouses within each group. Myocardial sleeve fibers were found extended from the left atrium to pulmonary vein from multiple directions. Significant difference of venoatrial wall and myocardial sleeve was found between old and middle-aged groups. Fibrosis,less elastic fibers,disorganized and thinner venous wall were seen in the sleeve of the old people.No significant difference in nerve density was found between all groups.Conclusion:The venoatrial architecture in normal pulmonary veins changes with ages,including circumference,myocardial sleeve and tissue constitution, which may be related to pathological change of AF and its ablation techniques.
