数字化设计/显示技术辅助经皮椎体后凸成形术的应用研究

摘    要:目的探讨数字化设计/显示技术辅助经皮椎体后凸成形术(D-PKP)对胸腰椎骨质疏松性椎体压缩骨折(OVCF)的临床疗效。方法选取2019年1月至2019年5月徐州医科大学附属淮安医院收治的25例采用经皮椎体后凸成形术(PKP)治疗的单节点高龄胸腰椎OVCF患者为研究对象,采用电脑随机抽样法随机分为数字化设计/显示技术辅助PKP组(D-PKP组)13例和常规PKP组(T-PKP组)12例。测量并比较2组患者手术时间、术中X射线透视次数、骨水泥灌注量、骨水泥与上/下终板最大接触比、骨水泥渗漏率、手术前后矢状位责任椎体后凸角、住院时间与术后12 h、1 d、1周、1个月的VAS和ODI,比较2组患者手术疗效。结果本研究2组患者骨水泥灌注量比较,差异无统计学意义(P>0.05);D-PKP组患者术后矢状位责任椎体后凸角、骨水泥与上/下终板最大接触比及骨水泥渗漏率明显优于T-PKP组,差异有统计学意义(P<0.05)。D-PKP组患者术中X射线透视次数少于T-PKP组,手术时间、住院时间短于T-PKP组,术后12 h、1 d的VAS和ODI优于T-PKP组,差异均具有统计学意义(P<0.05)。2组患者术后1周、1个月VAS与ODI比较,差异无统计学意义(P>0.05)。结论数字化设计/显示技术能够辅助PKP手术的精准实施,有助于提高OVCF的治疗效果。

Abstract:Objective To assess the outcome of digital design and display technology assisted percutaneous kyphoplasty(D-PKP)in treatment of osteoporosis vertebral compression fractures(OVCF).Methods From January to May 2019,25 elderly patients with single-node OVCF who were treated with PKP in Huai′an hospital affiliated to Xuzhou Medical University were selected.The patients were randomly divided into the digital design and display technology assisted PKP group(D-PKP group,13 cases)and the conventional PKP group(T-PKP group,12 cases)by computer random sampling.Measured and compared the operative time,number of intraoperative X-ray fluoroscopy,quantity of bone cement,contact ratio of bone cement and upper/lower endplate,rate of bone cement leaking,kyphotic angle of responsibile vertebral before and after surgery,length of hospital stay,VAS and ODI at 12 hours,1 day,1 week,1 month after surgery of the two groups.Results There was no significant difference in quantity of bone cement between the two groups.The kyphotic angle of responsibile vertebral,contact ratio of bone cement and upper/lower endplate,and rate of bone cement leaking of the D-PKP group were better than those in the T-PKP group,and differences were statistically significant(P<0.05).Patients of the D-PKP group had less intraoperative X-ray fluoroscopy,operative time and hospital stay compared with those of the T-PKP group.The VAS scores and ODI at 12 hour and 1 day after surgery were better than those in the T-PKP group,with statistically significant differences(P<0.05).On the contrary,there was no statistically significant difference in the VAS and ODI at 1 week and 1 month after surgery(P>0.05).Conclusion Digital design and display technology is helpful to keep the accurate implementation of PKP surgery and to improve the therapeutic effect of OVCF.
