非小细胞肺癌脑转移放射治疗的预后因素分析

目的探讨影响非小细胞肺癌脑转移全脑放射治疗预后的相关因素。方法回顾性分析该院2009年4月—2011年12月接受全脑放射治疗的88例非小细胞肺癌脑转移患者的临床资料,通过比例风险模型(Cox模型)进行多因素分析,生存分析采用Kaplan-Meier法并进行Log-rank时序检验,P0.05认为有统计学意义。结果单因素分析显示放射治疗前患者的KPS评分、肺癌原发灶控制情况、年龄、脑转移灶数目、分子靶向药物治疗情况及是否有颅外转移对生存期有影响(P0.05),多因素分析显示KPS评分、是否有颅外转移、肺癌原发灶控制及分子靶向药物治疗情况与预后有关(P0.05)。结论患者的KPS评分、是否有颅外转移、原发病灶控制及分子靶向药物治疗情况是非小细胞肺癌脑转移放射治疗预后的独立影响因素。

Analysis of prognostic factors in non-small cell lung cancer cases with brain metastasis by radiotherapy

Objective To evaluate the prognostic factors in non-small cell lung cancer(NSCLC) cases with brain metastasis(BM) by whole-brain radiotherapy(WBRT).Methods The clinical data of 88 non-small cell lung cancer cases with BM after WBRT in our hospital from Apr.2009 to Dec.2011 were retrospectively reviewed.The multivariate analysis was performed with Cox’s proportion risk model.Survival analysis was compared with Kaplan-Meier method and Log-rank test was used respectively.Statistical significance was defined as P<0.05.Results The univariate analysis showed that patients’ KPS score,control of primary tumor,age,the number of brain metastases,molecular targeted drug treatment,extracranial metastasis were related to survival period(P<0.05).The multivariate analysis indicated that patients’ KPS score,extracranial metastasis,control of primary tumor,molecular targeted drug treatment were closely related to their prognosis(P<0.05).Conclusion KPS score,extracranial metastasis,primary tumor control,molecular targeted drug treatment are the independent prognostic factors impacting the survival rate of non-small cell lung cancer with brain metastasis by whole brain radiotherapy.
