右美托咪啶在老年患者心脏手术中的临床应用

目的 探讨右美托咪啶临床应用于老年患者心脏手术中的效果。方法 采用随机对照试验，选取 2015 年7 月-2017 年6 月该院行心脏手术的老年患者，将其分为实验组和对照组。实验组采用右美托咪啶、 瑞芬太尼及丙泊酚联合行全身麻醉，对照组仅采用瑞芬太尼联合丙泊酚行全身麻醉；比较两组术中情况、手术 效果及麻醉药物使用情况。结果 实验组瑞芬太尼、丙泊酚的用量和输注速度均低于对照组（P <0.05）；实验 组SBP、DBP 及MAP 高于对照组，但HR 低于对照组。结论 相较于对照组，联合采用右美托咪啶行全身麻醉 可使术中患者血流动力学表现更加稳定，可减少瑞芬太尼和丙泊酚用量，对患者术后康复具有积极意义。

Clinical effect of Dexmedetomidine in elderly patients with cardiac surgery

Objective To investigate the clinical effect of Dexmedetomidine in elderly patients with cardiac surgery. Method With randomized controlled trial, the elderly patients who underwent cardiac surgeries from July 2015 to June 2017 in our hospital were divided into experimental group and control group. Dexmedetomidine, Remifentanil and Propofol were used in the experimental group for anesthesia while only Remifentanil and Propofol were used in the control group. Intraoperative conditions, surgical effects and the use of narcotic drugs of the two groups were compared. Result The dosage and infusion speed of Remifentanil and Propofol in the experimental group were lower than those in the control group. The systolic pressure, diastolic pressure and mean pressure in the experimental group were higher than those in the control group while the heart rate was lower. Conclusion Combined usage of Dexmedetomidine in anesthesia for patients with cardiac surgery can make the hemodynamic performance more stable to protect the myocardium and reduce the dosage of Remifentanil and Propofol and the side effects of the anesthetic drugs, which has a positive significance for postoperative rehabilitation of patients.
