糖化血红蛋白和尿微量白蛋白联合检测在 2 型糖尿病早期肾损伤中的临床价值

摘    要:目的:探讨糖化血红蛋白(HbA1c)和尿微量白蛋白(U-mAlb)联合检测对糖尿病早期肾损伤的临床意义。方法:采用高效液相色谱法、免疫比浊法检测145例2型糖尿病患者(2型糖尿病组)和58例健康体检者(正常对照组)的HbA1c和U-mAlb的水平,并按照HbA1c水平高低将145例2型糖尿病患者分为低值组(HbA1c〈7.0%)、中值组(7.0%≤HbA1c≤10.0%)和高值组(HbA1c〉10.0%),检测并比较U-mAlb在各组之间的关系。结果:糖尿病组HbA1c和U-mAlb结果明显高于正常对照组(P〈0.05);低、中、高值组U-mAlb结果比较差异有统计学意义(P〈0.05),且随着HbA1c水平的升高,其U-mAlb水平也升高。结论:糖尿病患者联合检测HbA1c和U-mAlb有利于糖尿病肾损伤的早期发现,为糖尿病患者的早期肾损害提供重要的临床依据。

Clinical Value of Glycosylated Hemoglobin and Microalbuminuria Combined Detection in the Early Renal Damage with Type 2 Diabetes Mellitus

Abstract:Objective:To discuss the clinical significance of glycosylated hemoglobin(HbA1c)and microalbuminuria(U-mAlb)combined detection in the diagnosis of early renal damage in diabetes. Method:The test of HbA1c was completed by High Performance Liquid Chromatography and immune turbidity method was used to detect U-mAlb .HbA1c and U-mAlb were mearsured in 145 patients with type 2 diabetes mellitus and 58 healthy individuals. According to HbA1c levels,the diabetes mellitus patients were divided into low(HbA1c〈7.0%),medium(7.0%≤HbA1c≤10.0%)and high value(HbA1c〉10%)groups. The differences of U-mAlb levels among the three groups were compared. Result:The results of HbA1c and U-mAlb in diabetic group were significantly higher than those of normal control group(P〈0.05). The results of U-mAlb were significantly different between the different groups of HbA1c level in diabetes mellitus(P〈0.05). The level of U-mAlb presented a rising trend with the increase of HbA1c. Conclusion:The combined detection of HbA1c and U-mAlb in patients with diabetes mellitus have found for early diabetic renal injury,to provide the clinical basis for early renal damage in patients with diabetes mellitus.
