129 例腔镜下甲状腺切除术后并发症的观察和护理

目的：探讨腔镜下甲状腺切除术后出现并发症的原因及有效的护理办法。方法：对本院近两年内129例腔镜下甲状腺切除术患者进行观察和护理。结果：129例患者术中大出血、中转开放1例，切口出血2例；皮下气肿2例；喉返或喉上神经损伤1例；呼吸困难1例；甲状旁腺损伤1例；甲状腺危象1例；胸前皮肤红肿、瘀斑2例；术中、术后均无死亡病例。结论：腔镜下甲状腺切除术较传统手术并不会增加手术的并发症；由于术后无明显的伤口，因此对护士提出了更高及更严格的要求，应及时观察患者的病情变化，最大范围内降低并发症发生后的危害，有效配合医师开展新技术创造有利条件。

The Observation and Nursing of Complications of 129 Patients after Thyroidectomy by Cavity Mirror

Objective： To investigate the cause and the effective nursing measures of complications after thyroidectomy under the endoscopy. Method： 129 patients were implemented the thyroidectomy in the laparoscopic and have been observationed and nursed caring at recently two years in our college. Result： All of 129 patients, there was 1 case of massive hemorrhage, transit open , 2 cases of incision bleeded, 2 cases of Subcutaneous emphysemaed, 1 case of recurrent laryngeal nerve or superior laryngeal nerve injured , 1 case of difficult breathed , 1 case of Parathyroid injured , 1 case of thyroid crisised , 2 cases of chest skin rednessed and swelled or ecchymosised, there were no deaths. Conclusion： Complications of laparoscopic resection of the thyroid is as much as traditional operation.There is no obvious postoperative wound; so it put forward higher and more stringent requirements to nurses, they should observe condition change of patients in time, reduce damage after complications at widest range, to develop new technology to create favorable conditions cooperating with doctors effectively.
