米非司酮配伍米索前列醇用于瘢痕子宫妊娠早期终止的疗效观察

目的：分析米非司酮配伍米索前列醇用于瘢痕子宫妊娠早期终止的效果。方法：选取2010年10月-2013年2月于本院妇产科进行瘢痕子宫妊娠早期终止的患者120例，随机分为对照组60例和观察组60例。观察组前3天早服80 mg，晚服60 mg的米非司酮，第4天服70 mg米非司酮和500μg米索前列醇，如不发动每4小时加用250μg米索前列醇，直至分娩；对照组前3天早服40 mg，晚服30 mg，第4天与观察组一样服用米索前列醇。统计两组患者完全流产人数、产后出血量、出血时间。结果：观察组患者完全流产人数（45例）和治疗有效率（93.3%）均高于对照组，差异均有统计学意义（P〈0.05）。观察组产后平均出血量为（74.3±10.7）mL，对照组产后平均出血量为（110.8±11.3）mL，两组产后出血量比较差异有统计学意义（P〈0.05）；观察组平均出血时间为（6.8±1.9）d；对照组平均出血时间为（12.8±2.1）d，观察组患者出血时间短于对照组，两组比较差异有统计学意义（P〈0.05）。对照组和观察组均有2例出现轻微胃肠道不良反应，经治疗后痊愈，无任何患者出现严重胃肠道和过敏反应的情况。结论：米非司酮配伍米索前列醇用于瘢痕子宫妊娠早期终止治疗效果显著，且大剂量米非司酮流产效果优于小剂量米非司酮，可以降低患者术后出血量和出血时间，且不会发生任何严重的不良反应，值得临床广泛推广使用。

Efficacy of Mifepristone and Misoprostol for Early Termination of Uterine Scar Pre gnancy

Objective：To analyze the effect of Mifepristone and misoprostol for early termination of uterine scar pregnancy.Method：120 cases of patients were randomly divided into two groups. The treatment group was taken does of 80 mg,60 mg of mifepristone in the first three days every morning and evening,the patients was taken 70 mg of meifepristone and 500μg of misoprostlol in the fourth day,if it is invalid plus 250μg misoprostol. The control group was taken does of 40 mg,30 mg of mifepristone in the first three days every morning and every evening,the patients was taken the same does in the fourth day as the treatment group.Statistics of two groups the number of complete abortion,postpartum blood loss,bleeding time. Result：The treatment group was better than the control group of patients with complete abortion number（45 cases）and the treatment efficiency（93.3%）,there was statistical significance（P〈0.05）.The treatment group of patients with（74.3±10.7）mL postpartum average blood loss and（6.8±1.9）d postpartum average bleeding time was better than the control group of patients with（110.8±11.3）mL postpartum average blood loss and（12.8±2.1）d postpartum average bleeding time,there was statistical significance（P〈0.05）.Conclusion：Mifepristone and misoprostol used in uterine scar pregnancy early termination of the treatment effect is remarkable,can decrease blood loss and postoperative bleeding time,and will not happen any serious adverse reactions,worthy of clinical use.
