食物特异性IgG与过敏性紫癜的相关性研究

摘    要:【目的】研究过敏性紫癜(HSP)与食物特异性IgG抗体水平相关性。【方法】选取HSP患儿155例作为试验组,无过敏史正常儿童36例作为对照组,采用酶联免疫吸附法(ELISA)检测两组儿童食物特异性IgG抗体水平,并比较几种不同情况下特异性IgG抗体阳性率:OHSP患儿与正常儿童;②有肾脏受累的HSP与无肾脏受累的HSP患儿;③有胃肠道受累的HSP与无胃肠道受累的HSP患儿。同时分析比较1gG抗体阳性的HSP患儿与IgG抗体阴性HSP患儿的临床各种参数的差异。【结果】①HSP患儿IgG抗体总体阳性显著率高于对照组(P〈0.05),且单项食物中牛奶,螃蟹,鸡蛋,虾IgG抗体阳性率高于对照组(P〈0.05)。②有肾脏受累与无肾脏受累的HSP患儿,IgG抗体阳性率无差别(P〉0.05)。③有胃肠道受累的HSP患儿IgG抗体阳挂率高于无胃肠道受累的HSP患儿(P〈0.05),且前者单项牛奶,鸡蛋的IgG抗体阳性率高于后者(Pd0.05),螃蟹特异性IgG阳性率显著高于后者(P〈0.01)。④IgG抗体阳性与IgG抗体阴性的HSP患儿相比较,在可能的诱因(上感,药物,食物,不明原因),发热,皮肤紫癜,关节受累,肾脏受累(血尿,蛋白尿,肾病综合征,血肌酐升高);白细胞升高,血沉升高,IgA升高等方面无差别(P〉0.05),但在胃肠道受累(腹痛,呕吐,黑便)等方面有差异(P〈0.05)。【结论】鸡蛋,牛奶,螃蟹,虾等特异性IgG抗体与过敏性紫癜关系密切,且可能通过影响胃肠系统功能而发挥作用。

Abstract:Objective]To explore the correlation between food allergen-specific IgG antibodies and Henoch-Schonlein purpura(HSP). Methods] A total of 155 children with HSP were taken as experimental group. Thirty six children without hypersensibility history were taken as control group. The food allergenspecific IgG antibodies in serum were measured by enzyme-linked immunosorbent assay(ELISA). The positive rates of allergen-specific IgG antibodies were compared between different groups. The clinical parameters between positive IgG antibodies HSP group and negative HSP group were compared. Results]The total positive rate of IgG antibody in HSP children was higher than that of the control group( P 〈0.05). The positive rate of IgG antibodies of simple food such as milk, crab, egg and shrimp was higher than that of control group( P〈0.05). There was no significant difference in the positive rate of IgG antibody between HSP groups with renal involvement and without renal involvement ( P〈0.05). The total positive rate of IgG antibody in HSP children with gastrointestinal involvement was higher than that of HSP children without gastrointestinal involvement( P 〈0.05). And the milk- or egg-specific IgG antibodies in the former were also higher than those in the latter( P 〈0.05). The crab specific IgG antibodies in former were significantly more than those in the latter( P 〈0.01). There was no difference in possible etiological factors such as upper respiratory tract infection, drugs, food, unknown origin, fever, Purpura, joint involvement, renal involvement (i. e. hematuria, proteinuria, nephrotic syndrome, elevated blood creatinine), elevated white blood cell, ESR and IgA between positive IgG antibody HSP group and negative HSP group, but there was difference in gastrointestinal involvement (i. e. stomachache, vomiting, melena)( P 〈0.05).
