同型半胱氨酸纤维蛋白原与冠心病的相关性分析

摘    要:目的 探讨冠心病(CHD)患者血浆中的同型半胱氨酸(HCY)、纤维蛋白原(FG)与CHD类型及冠状动脉病变程度的相关性.方法 对拟诊为冠心病的138例患者行冠状动脉造影(CAG)检查稳定性心绞痛35例(SAP组),不稳定心绞痛33例(UAP组),急性心肌梗死32例(AMI组),冠脉造影正常者38例(NC组)],测定患者血浆HCY,FG水平.结果 CHD各组患者血浆HCY水平与NC组相比非常显著增高(P<0.01),AMI、UAP、SAP3组HCY水平依次降低,差异有统计学意义(P<0.05);AMI组和UAP组患者血浆FG水平与NC组比较显著增高(P<0.05),而SAP组与NC组比较差异无统计学意义.单支病变、双支病变、三支病变组血浆HCY、FG水平与NC组比较差异均有统计学意义(P<0.05),冠心病各组两两比较中单支病变与双支病变组HCY、FG水平差异无统计学意义,而这两组与三支病变间比较有非常显著差异(P<0.01).结论 CHD患者HCY、FG水平与冠心病的病变程度和病变支数呈正相关.

The relationship between blood plasma homocysteine, FG level and the incidence of coronary heart disease

Abstract:Objective To explore the relationship between blood plasma homocysteine(HCY) , FG level and the incidence of coronary heart disease(CHD). Methods 138 patients who likely were CHD were performed coronary angiography, stable angina pcctoris (SAP) 35cases ; unstable angina pectoris (UAP) 33 cases ; acute myocardial infraction (AMI) 32 cases, coronary angiography negative(NC) 38 cases, the plasma HCY, FG levels of 4 groups were measured. Results To compare with controls, there were higher plasma levels of HCY in patients with CHD (P 〈 0. 01 ) , the plasma levels of HCY in AMI, UAP and SAP decreases in turn, there were significant difference between three groups ( P 〈 0. 05 ) ; there were higher serum levels of FG in AMI and UAP to compare with controls( P 〈0.05 ). there is no significant difference between SAP and controls. Compared with control, there were higher plasma levels of HCY and FG in patients with two vessel disease and multiple vessel disease( P 〈0.01 ) ,but the level of HCY and FG had no significant difference between single vessel disease and two vessel disease. Conclusion Plasma HCY and FG level are correlated to the extent of coronary artery lesion and the number of stenosed coronary artery vessel.
