原发性输尿管癌的MSCT诊断

摘    要:目的:探讨原发性输尿管癌的MSCT表现及其诊断价值。方法:回顾分析26例经病理证实的原发性输尿管癌患者的MSCT表现。所有病例进行MPR、CPR、部分病例加用VRT、MIP。结果:26例原发性输尿管癌CT平扫均表现为病变段输尿管壁增厚或结节状软组织密度影,MPR、CPR及VRT、MIP显示更清晰。增强扫描动脉期、静脉期病变持续强化,接近肾实质强化,延迟期强化程度减退,低于肾实质强化。病变以上输尿管不同程度扩张。结论:MSCT可准确显示输尿管癌的发生部位、病变形态、范围、病侧输尿管积水扩张和周围侵犯情况,增强扫描间接评价肾脏功能情况,为外科手术方案选择提供指导依据;MPR、CPR、VRT、MIP图像在显示病变全貌上具有显著优势。

Abstract:Objective; To evaluate the performance of primary ureteral cancer and diagnostic value ol MbU t. Metnoas: The MSCT menifestations of'26 cases confirmed by pathology in patients with primary ureteral carcinoma were retrospec- tively analyzed. All cases underwent routine multiplanar reconstruction (MPR), curved planar reconstruction (CPR), and in some cases subjected to with volume reconstruction (VRT), maximum intensity projection (MIP). Results..26 cases of primary ureteral carcinoma CT scan showed lesions as ureteral wall thickening or nodular soft tissue density. MPR and CPR and VRT and MIP showed more clearly. Enhanced CT in arterial phase, venous phase disease continued to strength- en, close to the renal parenchyma. The degree of enhancement in delay phase decreased, lower than the renal parenchyma. Different levels of expansion above the ureter lesion were obserred. Conclusion.. MSCT can accurately show the incidence of ureteral cancer site, extent of disease, morphology, disease side of the ureter and the surrounding water violation expan- sion. Enhanced scan can indirecly evaluac the contralateral kidney ipsilateral renal function, there hy providing guidance to choose surgery program basis MPR or CPR or VRT or MIP image has a significant advantage in showing lesions.
