幽门螺杆菌对慢性胃炎和消化性溃疡病致病性的探讨

对366例胃或(和)十二指肠球部粘膜进行观察并检测其幽门螺杆菌(HP)。结果HP的检出率分别为:慢性胃炎73.8％,胃溃疡病的溃疡旁粘膜88.2％,远离溃疡粘膜91.9％,球部粘膜11.1％。十二指肠溃疡病的窦部粘膜81.5％,球部溃疡旁粘膜24.6％,相对正常球部粘膜7.2％。活动性炎症HP明显高于非活动性炎症(P<0.05),但溃疡旁粘膜和远离渍疡粘膜无明显差异(P>0.05)。HP主要位于中性粘液中。作者认为HP是慢性胃炎尤其是活动性者的重要病因之一,也有可能促进消化性溃疡病的发生。本文还对HP的致病机理进行了讨论。

A Study on the Pathogenicity of Helicobacter pylori in Chronic Gastritis and Peptic Ulcer Disease

Gastroendoscopic biopsy spe-cimens from 366 patients were stained withHE, Warthin-Starrys or Giemsa and mucinhistochemical methods.Positive rate of He-licobacter pylori (HP)was 73.8% in chron-ic gastritis. Positive rates of HP in gas-tric. ulcer disease were 88.2%, 91.9%, and11.1% at the near and distant mucosa ofulcer and in duodenitis, respectively. Po-sitive rates of HP in duodenal ulcer dis-ease were 81.5%, 24.6% and 7.2% at thepyloric-antral area and at the near anddistant mucosa of duodenal ulcer, respec-tively. The number of HP in active inflam-mation was higherthan that in inactiveinflammation (P<0.05). The HP almostlived in the neutral mucin. There was nostatistical significant difference betweennear and distant mucosa of ulcer(P>0.05).HP might play an important role in thepathogenesis of chronic gastritis, and itmight aggravate the peptic ulcer disease.
