急性胰腺炎腹膜后隙的螺旋CT表现及其解剖基础

目的探讨急性胰腺炎腹膜后隙（Rs）的螺旋CT表现及其解剖基础。方法回顾分析118例急性胰腺炎螺旋CT表现，着重观察腹膜后隙的受累情况及其CT特征，以及与肾筋膜的关系。结果RS总的受累率为83．1％（98／118），APS、PS、PPS总的受累率分别为83．1％（98／118）、62．7％（74／118）、31．4％（37／118）。其中轻症急性胰腺炎44例，以累及APS为主，占54．5％（24／44）。重，型急性胰腺炎33例，以同时累及APS和PS两个间隙为主，占90．9％（30／33）。重2型急性胰腺炎41例，以同时累及APS、PS和PPS三个间隙为主，占80．5％（33／41）。结论螺旋CT可以全面而准确地显示AP累及RS的蔓延范围、程度和部位，AP越严重，累及的腹膜后间隙越多，腹膜后隙受累的程度越重。

Helical CT Featu res and Anatomic Basis of Retroperitoneal Space involvement in Acute Pancreatitis

Objective To evaluate helical CT features and anatomic basis ofretroperitoneal space involvement in acute pancreatitis .Methods Retrospectively analyzed helical CT manifestation of 118 cases in acute pancreatitis and focus on CT features ,incidence of RS involment,and its correlation with the renal fascia. Results The incidence of Retroperitoneal space involvement was 83.1%（98/I 18）.The incidence of anterior pararenal space, perirenal space, posterior pararenal space involvement were 83.1% （98/118）, 62.7 % （74/118）, 31.4 % （37/ 118）.In 44 cases of mild pancreatitis, 54.5 % （24/44） of them were involved with anterior pararenal space. In 33 cases of severe pancreatitis with grade one, 90.9 % （30/33） of them were involved with anterior pararenal space and perirenal space. In 41 cases of severe pancreatitis with grade two, 80.5 %（33/41） of them were involved with anterior pararenal space,perirenal space and posterior pararenal space.Conclusion Helical CT can show a full and accurate spread of RS, the extent and location involved in acute pancreatitis.and the more serious in AP, the more involving the retroperitoneal space,and the more serious degree of involvement.
