艾司西酞普兰与氢溴酸西酞普兰治疗抑郁症的对照观察

目的：探讨艾司西酞普兰对抑郁症患者的疗效和安全性。方法：68例符合CCMD-3抑郁发作的门诊患者随机分为艾司西酞普兰组和氢溴酸西酞普兰组，艾司西酞普兰剂量10～20mg·d^-1，氢溴酸西酞普兰20-50mg·d-^1，疗效采用汉密尔顿抑郁量表（HAMD）焦虑量表（HAMA），不良反应和安全性用TESS和实验室检查评定。观察时间为期6周。结果：艾司西酞普兰组有效率70．6％，治愈率47．1％，氢溴酸西酞普兰组有效率61．8％，治愈率41．2％，两组比较差异无显著性。艾司西酞普兰主要不良反应为食欲下降（11．8％）、恶心（8．8％）、头晕（5．9％）、口干（5．9％）等，与氢溴酸西酞普兰无差异。脱落率和失访率各为11．8％和14．7％，无显著差异。结论：艾司西酞普兰治疗抑郁症患者安全有效，疗效与氢溴酸西酞普兰相当，但起效快于氢溴酸西酞普兰。

Escitalopram versus Citalopram for Patients with Depressive Disorder

OBJECTIVE： To discuss the efficacy and safety of Escitalopram in the treatment of patients with depressive disorder. METHODS： A total of 68 outpatients who were diagnosed as having depression based on CCMD - 3 criteria were randomly assigned to receive escitalopram 10 - 20 mg· d^-1 （n = 34） or cital0pram 20 - 50 mg· d^-1 （n = 34）. Clinical efficacy was evaluated using HAMD and HAMA, and adverse reactions and safety were assessed with TESS and laboratory examination. The two groups were followed for 6 weeks. RESULTS ： The effective rate was 70.6 % and the cure rate was and 47. 1% for Escitalopram group versus 61.8% and 41.2% respectively for citalopram group, showing no significant differences between two groups. The main ADRs in Escitalopram - treated group were lack of appetite （ 11.8 % ）, nausea （8.8 % ）, dizziness （5.9 % ）, and dry mouth （5.9 % ）, nonsignificant as compared with Citalopram - treated group. The expulsion rate and the rate of loss of follow - up were 11.8% and 14.7%, respectively, showing no significance. CONCLUSION： Eseitalopram is safe and effective for patients with depressive disorder, and its curative efficacy is similar to Citalopram but it has more rapid onset of drug effect than the latter.
