纤维支气管镜用于小儿与成人喉罩置入后定位的对比研究

目的探讨小儿患者常规喉罩置入法喉罩理想解剖位置的到位情况。方法采用纤维支气管镜（fiberoptic bronchoscope,FOB）分别对40例使用喉罩全麻的小儿和40例成年患者验证喉罩置入位置的情况,在FOB直视下对位置欠佳原因进行分类和比较。结果位置分级小儿组明显低于成人组（P〈0.05）。小儿组喉罩位置不理想（位置分级≥2级）原因主要为会厌明显遮挡喉罩开口和置入过深;而成人组主要为喉罩置入深度不足。结论小儿喉罩的实际解剖到位率较成人低。

A comparison of fiberoptic bronchoscope for the localization of laryngeal mask airway between children and adults

Objective This study aims to investigative the conditions of ideal anatomic placement of the laryngeal mask airway（LMA） using the traditional blind insertion approach in pediatric patients.Methods 40 adult and 40 pediatric patients scheduled for elective surgery under general anesthesia were enrolled.After LMA was inserted,placements of LMA were examined and classification and comparison under direct vision by the use of a fiberoptic bronchoscopy（FOB）.Results Compared with the adults,in place of LMA was lower according to position classification（P0.05）.The inappropriate location of intubation of the main reasons（position classification≥2 grade） were that epiglottis significantly blocked the openings of LMA and LMA placed too deep in pediatric patients,but too low in adults.Conclusion Use of LMA in pediatric patients had worse localization than adults in anatomic place.
