婴幼儿先天性心脏病外科治疗的临床特点

目的:分析婴幼儿先天性心脏病外科治疗的临床特点。方法:总结婴幼儿先天性心脏病手术1276例的外科治疗情况。结果:男605例,女671例;年龄6天~3岁,平均13.6个月;体重2.2~18.0 kg,平均8.8 kg。1193例在全麻低温体外循环下行心脏畸形纠正术,83例采用深低温低流量体外循环。620例术中采用超滤技术。术后死亡17例(1.3%),死亡原因为低心排血综合征10例、肺动脉高压危象4例和肺部感染3例。结论:提高手术和体外循环技巧以及术后预防和治疗低心排血综合征和呼吸系统并发症的能力,是婴幼儿心脏手术成功的关键。

Surgical treatment of congenital heart disease in infants

Objective:To explore the experiences of the surgical treatment of congenital heart disease in infants.Methods:From Jan.2003 to Aug.2007,1 276 infants with congenital heart disease were operated in our hospital,their clinical data were retrospectively collected and analyzed.The patients′age ranged from 6 days to 3 years old(13.6 months).The body weight ranged from 2.2 to 18 kg(8.8 kg),1193 patients were operated under hypothermic anaesthesia with cardiopulmonary bypass(CPB),83 patients underwent deep thermal and low flow CPB.Ultrafiltration was used in 620 patients.Results:There were 17 deaths and the operative mortality was 1.3%,10 cases caused by low output syndromeclos(LOS),4 cases caused by pulmonary hypertension and 3 case caused by lung intection.Conclusion:To improve the operative and CPB technique,and to improve the skills of the postoperative managements of LOS and respiratory complications are the main points in the success of the cardiac operation in infants.
