血浆一氧化氮水平和脑血管反应性与进展性卒中的关系研究

摘    要:目的探讨一氧化氮(NO)和脑血管反应性(CVR)与进展性卒中(SIP)的关系。方法对发病24h内住院经头颅CT和/或MRI检查证实为大脑中动脉(MCA)供血区脑梗死患者,以斯堪的那维亚卒中量表(SSS)在住院72h内评分较入院时下降2分以上者判定为SIP患者,共50例;在住院72h内评分较入院时不下降或下降小于2分者判定为非SIP患者,共61例;以无脑梗死的体检者为正常对照组(52例)。采用硝酸还原酶法测定两组NO血浆水平,采用经颅多普勒超声结合屏气试验获取屏气指数(BHI)以评价CVR。结果 SIP组NO水平(40.15±4.73)显著低于非SIP组(56.55±8.35)和对照组(65.5±7.40)(P<0.00l),SIP组BHI(1.01±0.26)低于对照组(1.41±0.34)和非SIP组(1.18±0.30)(P<0.00l),BHI与NO血浆水平呈正相关(r=0.29,P=0.034)。结论 SIP患者的CVR降低与NO水平下降有相关性,CVR的下降是导致SIP的危险因素之一,检测NO水平和BHI有助于预测SIP发生的可能性,为临床治疗决策和预后判断具有重要意义。
