基于Meta分析探讨益气养阴活血法治疗室性早搏的疗效

目的:评价益气养阴活血法对室性早搏疗效及客观指标的影响,为临床运用提供循证依据。方法:检索中国知网、万方数据库、维普期刊资源整合服务平台等数据库中的随机对照试验。根据Jadad量化评分评价文献质量,采用Cochrane协作网提供的RevMan5.2软件进行Meta分析,对纳入文献包含的药物进行统计分析。结果:本研究共检索论文356篇,最终纳入15篇文献,观察病例1601例。与对照组比较,益气养阴活血类方药治疗室性早搏可以明显提高临床总有效率(RR=1.29,I^2=61%,P<0.00001)、改善室性早搏症状(RR=1.39,I^2=0%,P<0.00001)、提高心电图疗效(RR=1.37,I^2=0%,P<0.00001)、降低中医证候积分(SMD=-1.17,I2=79%,P<0.00001)、减少患者24h动态心电图中早搏次数(SMD=-0.93,I^2=26%,P<0.00001)、降低血清ET-1(SMD=-0.96,I^2=92%,P=0.03<0.05)、降低TXB 2(SMD=-1.42,I^2=0%,P<0.0001),活血化瘀类药物使用频率最高,其余依次为补气类、养阴类、安神类药物。结论:益气养阴活血法治疗室性早搏疗效显著,室性早搏多以气阴两虚夹瘀型多见。

Meta Analysis to Explore the Therapeutic Effect of Yiqi Yangyin Huoxue Method on Ventricular Premature Beats

Objective:To evaluate the effect of the method of replenishing qi,nourishing yin and activating blood on the curative effect and objective indicators of ventricular premature beats,and provide evidence-based basis for clinical application.Methods:Search for randomized controlled trials in databases such as CNKI,Wanfang Database,and VIP Journal Resource Integration Service Platform.The quality of the literature was evaluated according to the Jadad quantitative score,and the RevMan5.2 software provided by the Cochrane Collaboration was used for meta-analysis and statistical analysis of the drugs included in the literature.Results:A total of 356 papers were retrieved in this study,15 articles were finally included,and 1601 cases were observed.Compared with the control group,the treatment of ventricular premature beats by replenishing qi,nourishing yin and promoting blood circulation can significantly increase the total clinical effective rate(RR=1.29,I^2=61%,P<0.00001)and improve the symptoms of ventricular premature beats(RR=1.39,I^2=0%,P<0.00001),improve ECG curative effect(RR=1.37,I^2=0%,P<0.00001),reduce TCM syndrome score(SMD=-1.17,I^2=79%,P<0.00001),reduce patients by 24h The number of premature beats in Holter(SMD=-0.93,I^2=26%,P<0.00001),lower serum ET-1(SMD=-0.96,I^2=92%,P=0.03<0.05),lower TXB2(SMD=-1.42,I^2=0%,P<0.0001),the use of activating blood and removing blood stasis drugs is the highest,and the rest are qi,yin,and tranquilizing drugs in order.Conclusion:The method of replenishing qi,nourishing yin and promoting blood circulation is effective in treating ventricular premature beats,and ventricular premature beats are more common in qi and yin deficiency and stasis.
