MRI对急性胰腺炎的早期诊断价值

目的分析急性胰腺炎（acute pancreatitis,AP）的MRI和CT表现,提高急性胰腺炎的早期诊断水平,并为临床提供治疗依据。方法选择在我院确诊为急性胰腺炎并且资料完整的33例患者,在24 h内完成上腹部MRI平扫、CT平扫及增强检查。根据Balthazar分级,分别对急性胰腺炎进行CT、MRI分级。结果CT、MRI对急性胰腺炎分级差别主要表现在A级和B级,其中6例CT表现正常的AP,因MR-T2WI表现为高信号而分为MRI B级,两者在A、B级分级上差别具有统计学意义;而在C、D、E级AP的分级上MRI和CT分级基本一致。结论MRI平扫可以对急性胰腺炎进行准确分级,并有利于急性胰腺炎的早期诊断。

The early diagnosing value of MRI on acute pancreatitis

Objective To analyze the manifestation of acute pancreatitis（AP）on CT and MRI for improving the early diagnosing value in acute pancreatitis and to provide a clinical basis of treatment.Methods Abdomen nonenhanced MR imaging and CT contrast scan were performed within 24 hours after admission in 33 acute pancreatitis patients.Acute pancreatitis grading was achieved on CT and MRI with Balthazar Classification.Results The maximal difference was found between grade A and grade B pancreatitis on CT and MRI.Six grade A acute pancreatitis patients on CT were reclassified to grade B because they showed high signal on MR T2-weighted sequences.Although there was significant difference between grade A and grade B on CT and MRI,general accord of grade C to grade E was determined.Conclusion Nonenhanced MR imaging may be regarded as a key standard to classify acute pancreatitis,and it may help early diagnoseing acute pancreatitis.
