甲亢糖代谢紊乱与红细胞胰岛素受体的改变

摘    要:对未经治疗的Graves病人20例及正常人20例进行血糖、胰岛素、胰岛素受体检测。Graves病人均有不同程度高胰岛素血症(P<0.01)。甲亢OGTT异常组红细胞胰岛素受体的四个参数Q1、K1、Q2、K2均减低,后两个参数与正常组比较有显著性差异(P<0.01),且甲亢OGTT异常组餐后2小时血糖与K2值呈显著负相关(r=-0.4190,P<0.05)。提示甲亢糖代谢异常可能是由于胰岛素受体的结构改变,使胰岛素与IR的结合位点数减少及亲合力降低而致胰岛素抵抗引起。

Abstract:The blood glucose levels,plasma insulin and erythrocyte insulin receptors in 20 patients with untreated hyperthyroidism and 20 healthy controls were collected.There were hyperinsulinemia in all patients with hyperthyroidism.The four parameters Q1,K1,Q2,K2 of erythrocyte insulin receptors were decreased in hyperthyroidism with abnomal OGTT group,The latter two parameters in abnomal OGTT group were significantly lower than in control group(P<0.01).There was a significant negative correlation between two-hour postprandial blood glucose level and K2 in abnormal OGTT group(r=r-0.4190,P<0.05).The results suggest that the abnormal glucose metabolism may be due to structural changes of the insulin receptors causing the decrease of binding sites and affinity to insulin and resulting in insulin resistance.
